摘要
目的:研究心源性应激保护的实施方法及在CCU重症左心衰护理中的应用效果。方法:将某院CCU收治的90例重症左心衰患者随机分为C组和Y组各45例,C组采用一般性护理,Y组采用一般性护理+心源性应激保护干预(主要包括改善心源性呼吸困难、减轻心脏负荷、积极预防心脏并发症、加强心脏康复)。于护理干预前后,检测患者血清心肌损伤标志物[B型脑利钠肽前体(NT-proBNP)、肌钙蛋白T(cTnT)、心肌酶(CK-MB)]、心功能指标[心脏指数(CI)、心输出量(CO)、心搏指数]和血气指标(PaO_(2)、PaCO_(2)、SaO_(2))。结果:护理后,Y组患者血清NT-proBNP、cTnT、CK-MB均低于C组(P<0.05),Y组CI、CO、心博指数均高于C组(P<0.05),Y组患者PaO_(2)、SaO_(2)均高于C组(P<0.05),PaCO_(2)低于C组(P<0.05)。结论:于CCU重症左心衰患者救治中通过心源性应激保护干预能有效减轻患者心肌损伤程度,提高心功能,改善血气指标。
Objective:To study the implementation method of cardiogenic stress protection and its application effect in the nursing of severe left heart failure in CCU.Methods:90 patients with severe left heart failure admitted to CCU of a hospital were randomly divided into C group(n=45)and Y group(n=45),and the two groups were given general nursing and general nursing+cardiogenic stress protection intervention(mainly including improving cardiogenic dyspnea,reducing cardiac load,actively preventing cardiac complications,and strengthening cardiac rehabilitation),respectively.Before and after nursing intervention,serum myocardial injury markers[B-type brain natriuretic peptide precursor(NT-proBNP),troponin T(cTnT),myocardial enzyme(CK-MB)],cardiac function indicators[cardiac index(CI),cardiac output(CO),stroke index]and blood gas indicators(PaO_(2),PaCO_(2),SaO_(2))were detected.Results:After nursing,serum NT-probNP,cTnT and CK-MB in Y group were lower than those in C group(P<0.05),CI,CO and cardiac index in Y group were higher than those in C group(P<0.05),PaO_(2) and SaO_(2) in Y group were higher than those in C group(P<0.05),and PaCO_(2) was lower than that in C group(P<0.05).Conclusion:Cardiogenic stress protection intervention can effectively reduce the degree of myocardial injury,improve cardiac function and improve blood gas indexs in CCU patients with severe left heart failure.
