摘要
目的:探讨C反应蛋白(CRP)、降钙素原(PCT)与免疫指标检测用于早期诊断膝关节置换术后感染的应用价值。方法:选取2020年1月至2021年1月接收的40份膝关节置换术的血液标本,根据术后是否合并早期感染分成感染组(n=16)与非感染组(n=24),均采集患者的血液指标,比较患者术前、术后1.d、术后3.d、术后7.d的CRP、PCT与免疫指标[免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)],并分析单项指标测定与联合测定诊断术后早期感染的价值。结果:感染组术后3.d、7.d血清CRP与PCT水平显著高于非感染组(P<0.05);感染组术后1.d、3.d、7.d的血清IgA、IgG、IgM水平显著低于非感染组(P<0.05);CRP、PCT与免疫指标联合检测的灵敏度、特异度与曲线下面积分别为90.22%、60.21%、0.913,均显著高于单一指标检测。结论:膝关节置换术后,早期感染可升高患者血清CRP与PCT水平,降低免疫学指标,而采取CRP、PCT与免疫学指标联合检测方式对诊断膝关节置换术后早期感染的价值突出,值得推广。
Objective:To explore the application value of C-reactive protein(CRP),procalcitonin(PCT)and immune index detection in early diagnosis of infection after knee arthroplasty.Methods:40 blood samples of knee arthroplasty from January 2020 to January 2021 were selected and divided into infection group(n=16)and non-infection group(n=24)according to whether early infection occurred after operation.The blood indexes of patients were collected.CRP,PCT and immune indexes[immunoglobulin A(IgA),immunoglobulin G(IgG),and immunoglobulin M(IgM)]before operation,1 d after operation,3 d after operation,and 7 d after operation were compared.In addition,the value of single index determination and combined determination in the diagnosis of early infection after operation was analyzed.Results:The levels of serum CRP and PCT in the infection group were significantly higher than those in the non-infection group at 3 d and 7 d after operation(P<0.05).The levels of serum IgA,IgG and IgM in the infection group were significantly lower than those in the non-infection group at 1 d,3 d and 7 d after operation(P<0.05).The sensitivity,specificity and area under curve of combined detection of CRP,PCT and immune indexes were 90.22%,60.21%and 0.913,respectively,which were significantly higher than those of single index detection.Conclusion:After knee arthroplasty,early infection can increase serum CRP and PCT levels and reduce immunological indexes.The combined detection of CRP,PCT and immunological in-dexes is of great value in the diagnosis of early infection after knee arthroplasty,which is worthy of promotion.
