摘要
目的分析多层螺旋CT(MSCT)灌注成像对重症急性胰腺炎的临床诊治价值。方法收集本院2017年3月至2019年7月收治的95例急性胰腺炎患者的临床资料,其中MAP患者47例,SAP患者48例。另选取同期在本院进行MSCT检查,非急性胰腺炎患者57例。对患者所得图像进行分析,对比不同人群MSCT灌注参数差异。结果非急性胰腺炎患者、MAP、SAP三者比较中BF、BV、PS参数值存在明显差异(P<0.05),BF、BV值均以SAP最低,其次为MAP患者和非急性胰腺炎患者,PS参数值从高到低依次为SAP患者、MAP患者、非急性胰腺炎患者。三者在MTT参数比较上无差异(P>0.05);MSCT表现:MSCT可见水肿型患者胰腺出现弥漫性肿大,密度尚均匀,边缘较清楚,周围脂肪间隙立刻消失,周围无明显液体渗出。出血坏死型可见患者病灶密度不均匀,周围脂肪层模糊,周围有液体渗出。结论MSCT灌注成像对SAP的临床诊治价值高,可为其临床诊断以及病情评估提供参考依据,有效降低SAP病死率。
Objective To analyze the clinical value of multi-slice spiral CT(MSCT)perfusion imaging in severe acute pancreatitis.Methods The clinical data of 95 patients with acute pancreatitis collected in our hospital from March 2017 to July 2019 were collected,of which 47 were MAP patients and 48 were SAP patients.In addition,another 57 patients without acute pancreatitis who underwent MSCT in our hospital during the same period were selected.The images of the patients were analyzed to compare the differences in MSCT perfusion parameters in different people.Results There were significant differences in the values of BF,BV,and PS among patients without acute pancreatitis,MAP patients,and SAP patients(P<0.05).The BF and BV values were the lowest in SAP patients,followed by MAP patients and patients without acute pancreatitis.The values of PS parameters the highest in SAP patients followed by MAP patients,and patients without acute pancreatitis.There was no difference in the MTT parameters among the patients(P>0.05).MSCT manifestations:MSCT showed diffuse swelling of the pancreas in edema patients,the density was uniform,the edges were clearer,the surrounding fat space immediately disappeared,and there was no obvious fluid leakage around it.The hemorrhagic necrosis type shows that the density of the lesions was uneven,the surrounding fat layer was blurred,and the surrounding fluid exuded.Conclusion The value of MSCT perfusion imaging in the clinical diagnosis and treatment of SAP is high,which can provide a reference for its clinical diagnosis and evaluation of condition,and effectively reduce mortality of SAP.
