摘要
目的分析胰腺黏液性囊性肿瘤(MCN)患者CT特征及良、恶性影响因素。方法回顾性分析2017年1月至2019年8月本院收治的43例MCN患者,均经手术病理诊断,其中良性MCN以及恶性MCN分别有28例、15例。对患者所得图像进行分析,比较良、恶性MCN的CT影像学征象,并分析影响MCN性质的危险因素。结果良性MCN与恶性MCN在CT真相上存在明显差异(P<0.05),良性MCN肿瘤部位为胰尾部,肿瘤边界清晰,囊壁薄(≤0.2cm),主胰管扩张情况少;恶性MCN肿瘤部位为胰头部,肿瘤边界多不清晰,囊壁厚(>0.2cm),肿瘤内存在实性成分,并伴有主胰管扩张情况出现;通过单因素分析有差异者为自变量进一步行多因素分析,Logistic多因素结果显示:囊壁厚度>0.2cm,囊肿内存在实性成分为判断恶性MCN的独立因素(P<0.05)。结论MCN在CT征象上存在一定特异性,肿瘤伴有实性成分、囊壁厚度>0.2cm则高度为发生恶性MCN的独立因素,CT影像学表现对MCN患者诊断及手术方案有重要指导意义。
Objective To analyze the CT features of patients with mucinous cystic neoplasm of pancreas(MCN)and the influencing factors of benign and malignant tumors.Methods The clinical data of 43 patients with MCN admitted to our hospital from January 2017 to August 2019 were retrospectively analyzed,and all were diagnosed by pathology.Among them,tumors of 28 patients were benign and tumors of 15 patients were malignant.The images of the patients were analyzed,the CT imaging signs of benign and malignant MCN were compared,and the risk factors affecting the nature of MCN were analyzed.Results There are significant differences in CT signs between benign MCN and malignant MCN(P<0.05).Benign MCN tumors were located in the tail of the pancreas,with clear boundaries of tumor,thin cystic walls(>0.2cm),and the dilatation of main pancreatic ductis rare.Malignant MCN tumors were located in the head of the pancreas.The boundaries of the tumors were often unclear,with thick cystic wall(≤0.2cm).There were solid components in the tumor,accompanied by the dilatation of the main pancreatic duct.Those who had differences through univariate analysis were used as independent variables for further multivariate analysis.Logistic multivariate results showed that the thickness of the cyst wall was≤0.2cm,and the presence of solid components in the cyst was independent factor for judging malignant MCN(P<0.05).Conclusion MCN has some specificity in CT signs.Tumors with solid components and thickness of cyst wall>0.2cm are highly independent factors for the occurrence of malignant MCN.CT imaging signs have important guiding significance for the diagnosis and surgical plan of MCN patients.
