摘要
目的:系统研究参苓白术散加减治疗小儿腹泻的疗效与安全性,为参苓白术散应用于小儿腹泻提供循证依据。方法:两名研究者在CNKI、WanFang Data、VIP、CBM、PubMed、EMbase和The Cochrane Library数据库进行检索,从建库截止至2021年3月18日,按纳排标准,筛选出符合要求的随机对照研究(randomized controlled trials,RCTs)。对纳入的研究进行偏倚风险的评估、提取相关数据信息,并进行Meta分析。结果:共纳入13篇符合纳入标准的随机对照研究,结果表明与西医治疗或常规治疗的对照组相比,参苓白术散加减可提高腹泻患儿的临床总疗效率[RR=1.21,95%CI(1.16-1.27),P<0.00001]和临床疗效愈显率[RR=1.33,95%CI(1.19-1.49),P<0.00001],观察组的不良事件发生率较低。结论:参苓白术散加减在治疗小儿腹泻中具有良好疗效,可提高患儿临床总疗效率、临床疗效愈显率,降低不良反应,但在临床服用时应注意服用剂量及服用时间。由于纳入的原始研究数量较少、质量普遍不高,原始研究的样本量较小等原因,本研究得出的结论尚需要更多高质量、大样本的随机对照原始研究进行验证。
Objective To systematically study the efficacy and safety of Shenling Baizhu Powder in the treatment of infantile diarrhea,and to provide evidence-based basis for the application of Shenling Baizhu Powder in infantile diarrhea.Methods Two researchers searched CNKI,Wanfang Data,VIP,CBM,PubMed,Embase and The Cochrane Library databases since its establishment until March 18,2021.Randomized controlled trials(RCTs)were selected to meet the requirements.The included studies were assessed for the risk of bias,relevant data information was extracted,and meta-analysis was performed.Results A total of 13 randomized controlled studies that met the inclusion criteria were included.The results showed that compared with the control group of western medicine or conventional treatment,Shenling Baizhu Powder could improve the total clinical efficacy rate of children with diarrhea[RR=1.21,95%CI(1.16-1.27),P<0.00001]and the recovery rate of clinical efficacy[RR=1.33,95%CI(1.19-1.49),P<0.00001],the incidence of adverse events in observation group was lower.Conclusion Shenling Baizhu Powder has a good effect in the treatment of children with diarrhea,can improve the total clinical effect rate and clinical curative effect recovery rate of children with diarrhea,reduce adverse reactions.When taking,pay attention to the dosage and time.Due to the small number and generally low quality of the included original studies and the small sample size of the original studies,the conclusions drawn in this study still need to be verified by more high-quality and large-sample randomized controlled original studies.
