摘要
目的:分析中小剂量丙种球蛋白治疗重症免疫性血小板减少性紫癜(ITP)患儿的临床疗效。方法:选择2019年2月-2021年3月泰安市妇幼保健院儿科收治的62例重症ITP患儿,随机分为两组,对照组行大剂量丙种球蛋白进行治疗、观察组行中小剂量丙种球蛋白进行治疗。比较两组患儿治疗效果、血小板参数、症状消退时间、治疗费用和不良反应。结果:两组患儿治疗有效率比较,差异无统计学意义(P>0.05);两组患儿治疗前的血小板计数(PLT)、血小板比容、血小板平均体积、血小板分布宽度水平,差异无统计学意义(P>0.05),治疗后均较治疗前有明显升高,差异有统计学意义(P<0.05);且治疗后两组,差异无统计学意义(P>0.05);两组患儿的PLT恢复正常时间、出血停止时间,差异无统计学意义(P>0.05);但观察组的治疗费用少于对照组,差异有统计学意义(P<0.05);不良反应发生率,观察组与对照组比较,差异无统计学意义(P>0.05)。结论:中小剂量丙种球蛋白治疗重症ITP,能够获得与大剂量丙种球蛋白相当的疗效,且治疗服用更低,具有较高的临床推广应用价值。
Objective:To analyze the clinical efficacy of small-and medium-dose gamma globulin in the treatment of children with severe immune thrombocytopenic purpura(ITP).Methods:A total of 62 children with severe ITP admitted to Department of Pediatrics,Taian Maternity and Child Health Hospital from February 2019 to March 2021 were selected and randomly divided into two groups.The control group was treated with high-dose gamma globulin and the observation group was treated with mediumand small-dose gamma globulin.The treatment effect,platelet parameters,symptom subsidence time,treatment cost and adverse reactions were compared between the two groups.Results:There was no statistically significant difference in the treatment effec⁃tive rate between the two groups(P>0.05);there was no statistically significant difference in the levels of platelet count(PLT),plateletcrit,mean platelet volume,and platelet distribution width between the two groups before treatment(P>0.05),after treat⁃ment these levels were significantly higher than those before treatment,and the difference was statistically significant(P<0.05);and after treatment,there was no statistically significant difference between the two groups(P>0.05);there was no statistically sig⁃nificant difference in the recovery time of PLT and the stop time of bleeding in children of two groups(P>0.05);however,the treat⁃ment cost of the observation group was less than that of the control group,and the difference was statistically significant(P<0.05);there was no statistically significant difference in the incidene of adverse reaction between the observation group and the control group(P>0.05).Conclusion:Medium-and small-dose gamma globulin in treatment of severe ITP can obtain the same curative ef⁃fect as high-dose gamma globulin,and the therapeutic dosage is lower,it has high value of clinical promotion and application.
