摘要
目的应用盆底超声技术评估椎管内阻滞分娩镇痛对初产妇产后早期盆底功能的影响。方法回顾性分析70例经阴道分娩初产妇的临床资料,根据是否使用椎管内阻滞分娩镇痛将产妇分为观察组和对照组,各35例。两组产妇均于产后6周行盆底超声检查,比较两组受检者的膀胱颈移动度(BND)、最大Valsalva状态下的膀胱尿道后角(PUVA)和肛提肌裂孔面积,以及最大Valsalva状态下尿道内口漏斗化的发生率。结果观察组产妇BND和最大Valsalva状态下的肛提肌裂孔面积和PUVA均小于对照组,且最大Valsalva状态下的尿道内口漏斗化发生率低于对照组(均P<0.05)。结论采用椎管内阻滞分娩镇痛对初产妇产后早期的盆底功能具有一定的保护作用,盆底超声技术能为初产妇产后盆底功能检查提供可靠的影像学依据。
Objective To investigate the effect of the evaluation on intraspinal block in labor analgesia using the pelvic floor ultrasound technique on postpartum early pelvic floor function in primiparae.Methods The clinical data of 70 primiparae with transvaginal delivery were retrospectively analyzed.The primiparae were divided into observation group(n=35)and control group(n=35)according to whether they had received intraspinal block in labor analgesia.And primiparae in both groups underwent pelvic floor ultrasound examinations six weeks after delivery.The subjects′bladder neck descent(BND),posterior urethro-vesical angle(PUVA)and levator hiatus(LH)area on the maximum Valsalva maneuver,as well as the incidence rate of internal urethral orifice funnelation on the maximum Valsalva maneuver,were compared between the two groups.Results Primiparae in the observation group had a decreased BND,smaller LH area and PUVA on the maximum Valsalva maneuver,and a higher incidence rate of internal urethral orifice funnelation on the maximum Valsalva maneuver as compared with the control group(all P<0.05).Conclusion The use of intraspinal block in labor analgesia has a certain protective effect on postpartum early pelvic floor function in primiparae.The pelvic floor ultrasound technique can provide a reliable imaging basis for postpartum pelvic floor function examination in primiparae.
