摘要
阻塞性睡眠呼吸暂停低通气综合征(OSAHS)是睡眠时反复出现的呼吸暂停和低通气导致的临床综合征,以低氧血症、高碳酸血症、睡眠质量下降等引起多器官功能障碍为主要特征。近年来,临床发现OSAHS患者常常患有勃起功能障碍(ED)。目前,关于OSAHS并发ED的相关机制尚不明确,可能与睾酮水平下降、一氧化氮及一氧化氮合酶减少、神经损伤、肿瘤坏死因子-α升高有关。临床治疗以持续气道正压通气和5型磷酸二酯酶抑制剂为主,手术治疗和运动减肥作为补充。近年来,新的治疗方式不断丰富,精准治疗、个体化治疗成为主流。现对其相关的发病机制及临床治疗最新进展进行综述。
Obstructive sleep apnea hypopnea syndrome(OSAHS)is a clinical syndrome caused by recurrent apnea and hypopnea during sleep,which is mainly characterized by multiple organ dysfunction caused by hypoxemia,hypercapnia and decreased sleep quality.In recent years,it has been found that patients with OSAHS often suffer from erectile dysfunction(ED).At present,the related mechanism of OSAHS complicated with ED is not clear,which may be related to decreased testosterone level,reduction of NO and NO synthase,nerve injury and increase of tumor necrosis factor-α.Clinical treatment is based on continuous positive airway pressure and phosphodiesterase inhibitor 5,supplemented by surgical treatment and exercise weight loss.In recent years,new treatment methods are increasingly abundant,and precision therapy and individualized therapy have become the mainstream.This article reviews the latest progress of pathogenesis and clinical treatment.
