病原宏基因组测序对肺部感染病原体检测价值以及影响因素分析 被引量:5

Analysis on the value and influencing factors of pathogen metagenomic sequencing in the detection of lung infection pathogens

摘要 目的探讨mNGS对肺部感染病原体检测价值以及影响因素分析。方法收集2021年1月-2021年6月本院住院并送检mNGS的肺部感染者19例,记录患者临床资料、mNGS结果以及传统病原学检测结果。结果mNGS检测阳性率为73.68%,显著高于传统培养(47.37%,P<0.05);mNGS检测出2种及2种以上的病原体比例为52.63%,主要为铜绿假单胞菌、流感嗜血杆菌、人类疱疹病毒4型、人类疱疹病毒7型、细环病毒、星座链球菌、肺炎链球菌和白色念球菌。多因素Logistic回归分析显示未发现影响mNGS检测阳性率的因素。结论mNGS对肺部感染病原体检出率高,受干扰小,具有很高的临床应用价值。 Objective To explore the detection value of pathogen metagenomic sequencing(mNGS)for lung infection pathogens and analysis of influencing factors.Methods It collected 19 cases of pulmonary infection who were hospitalized in our hospital from Jan.2021 to Jun.2021 and submitted for mNGS examination,and recorded the patient's clinical data,mNGS results and traditional etiological test results.Results The positive rate of mNGS detection was 73.68%,which was significantly higher than that of traditional detection(47.37%,P<0.05);The pathogens detected by mNGS were mostly two or more types,accounted for 52.63%,and the main pathogen were pseudomonas aeruginosa spore bacteria,haemophilus influenzae,human herpes virus type 4,human herpes virus type 7,parvovirus,streptococcus constellation,streptococcus pneumoniae and candida albicans.Multivariate logistic regression analysis showed that no factors affecting the positive rate of mNGS detection were found.Conclusions mNGS has a high detection rate of pathogens in lung infections and little interference,which has high clinical application value.
