摘要
目的:比较皮肤软组织扩张术与复合皮移植术治疗烧伤后瘢痕的效果。方法:抽取2018年1月至2020年12月南石医院收治的烧伤后瘢痕患者78例,按随机数字表法分为扩张组(39例)与移植组(39例),扩张组采用皮肤软组织扩张术治疗,移植组采用复合皮移植术治疗。比较两组疗效、皮肤恢复情况、温哥华瘢痕量表(VSS)评分、观察者瘢痕评估量表(OSAS)评分、中文版精简烧伤健康量表(BSHS-B)评分、并发症发生率、美观满意度。结果:扩张组总有效率为94.87%(37/39),较移植组的76.92%(30/39)高, P<0.05;扩张组皮肤血运恢复、创面愈合时间较移植组短( P<0.05);扩张组厚度、血管分布、柔软度、色泽评分较移植组低( P<0.05);扩张组OSAS评分较移植组低,BSHS-B评分较移植组高( P<0.05);扩张组并发症发生率为5.13%(2/39),低于移植组的23.08%(9/39), P<0.05;扩张组美观满意度为92.31%(36/39),低于移植组的74.36%(29/39), P<0.05。 结论:与复合皮移植术比较,皮肤软组织扩张术治疗烧伤后瘢痕患者效果确切,可加快皮肤恢复,改善瘢痕情况,并可降低并发症发生率,提高美观度。
Objective To compare the effects of skin and soft tissue expansion and composite skin grafting in the treatment of postburn scar.Methods A total of 78 patients with postburn scars in Nanyang Nanshi Hospital from January 2018 to December 2020 were selected and divided into expansion group(39 cases)and transplantation group(39 cases)according to the random number table method.The expansion group was treated by skin and soft tissue expansion,and the transplantation group was treated by composite skin grafting.The efficacy,skin recovery,Vancouver Scar Scale(VSS)score,Observer Scar Assessment Scale(OSAS)score,Chinese simplified Burn Specific Health Scale-Brief(BSHS-B)score,complication rate,and aesthetic satisfaction were compared between the two groups.Results The total effective rate of the expansion group was 94.87%(37/39),which was higher than 76.92%(30/39)of the transplantation group,P<0.05.The skin blood supply recovery and wound healing time in the expansion group were shorter than those in the transplantation group(P<0.05).The thickness,blood vessel distribution,softness,and color scores of the expansion group were lower than those of the transplantation group(P<0.05).The OSAS score of the expansion group was lower than that of the transplantation group,and the BSHS-B score was higher than that of the transplantation group(P<0.05).The complication rate of the expansion group was 5.13%(2/39),which was lower than 23.08%(9/39)of the transplantation group(P<0.05).The shape satisfaction degree of the expansion group was 92.31%(36/39),which was higher than 74.36%(29/39)of the transplantation group(P<0.05).Conclusions Compared with composite skin grafting,skin and soft tissue expansion is effective in the treatment of postburn scar.It can speed up skin recovery,improve scars,reduce the incidence of complications,improve aesthetics.
