摘要
目的探讨MRI对于扩张型心肌病(DCM)诊断、病情评估的应用价值。方法选择2018年1月至2020年12月在内蒙古自治区国际蒙医医院收治的DCM患者65例(观察组),其中男性41例,女性24例;年龄25~73岁,平均年龄49.52岁;纽约心脏病协会(NYHA)心功能分级Ⅰ级12例,Ⅱ级19例,Ⅲ级21例,Ⅳ级13例。另选取同期在医院体检的健康者50例(对照组),其中男性32例,女性18例;年龄22~72岁,平均年龄49.27岁。两组均接受MRI检查,测量左心室舒张末期室壁厚度(TDD)、左心室收缩末期室壁厚度(TSD)及左心室心肌质量(LVMM),计算左心室舒张末期容积(LVEDV)、收缩末期容积(LVESV)、射血分数(LVEF)等参数。结果观察组TDD、TSD、LVEF小于对照组[(4.82±0.95)mm vs(6.53±1.23)mm、(3.34±0.58)mm vs(5.77±0.83)mm、(31.78±5.96)%vs(66.26±6.15)%。P<0.05],LVMM、LVEDV、LVESV大于对照组[(157.28±28.47)g vs(124.86±23.58)g、(242.24±43.85)mL vs(84.26±15.68)mL、(183.92±34.55)mL vs(36.94±7.24)mL。P<0.05];心功能Ⅰ~Ⅱ级患者LVEDV、LVESV小于心功能Ⅲ级患者,心功能Ⅲ级患者小于Ⅳ级患者;心功能Ⅰ~Ⅱ级患者LVEF大于心功能Ⅲ级患者,心功能Ⅲ级患者大于Ⅳ级患者(P<0.05);LVEDV、LVESV与心功能分级存在正相关(r=0.583、0.734,P<0.01),LVEF与心功能分级存在负相关(r=-0.782,P<0.01)。结论MRI参数与DCM患者NYHA心功能分级显著相关,能够有效评估DCM病情。
Objective To investigate the application value of MRI in diagnosis and evaluation of dilated cardiomyopathy(DCM).Methods From January 2018 to December 2020,a total of 65 patients with DCM(observation group)were enrolled,which included 41 males and 24 females,aged 25-73 years old with mean age of 49.52 year old.According to New York Heart Association(NYHA)cardiac function classification,three were 12 cases of gradeⅠ,19 of gradeⅡ,21 of gradeⅢand13 of gradeⅣ.Meantime,50 healthy of physical examination were set as control group,which included 32 males and 18 females,aged 22-72 years old with mean age of 49.27 years old.All of them performed MRI examination,the left ventricular end-diastolic wall thickness(TDD),left ventricular end-systolic wall thickness(TSD)and left ventricular myocardial mass(LVMM)were measured,and left ventricular end-diastolic volume(LVEDV),left ventricular ejection fraction end-systolic volume(LVESV)and left ventricular ejection fraction(LVEF)were calculated.Results The TDD,TSD and LVEF in observation group were smaller than those in control group[(4.82±0.95)mm vs(6.53±1.23)mm,(3.34±0.58)mm vs(5.77±0.83)mm,(31.78±5.96)%vs(66.26±6.15)%.P<0.05],but LVMM,LVEDV and LVESV were higher than those in control group[(157.28±28.47)g vs(124.86±23.58)g,(242.24±43.85)mL vs(84.26±15.68)mL,(183.92±34.55)m L vs(36.94±7.24)m L.P<0.05].The LVEDV and LVESV in patients with cardiac function gradeⅠ-Ⅱwere lower than those in patients with cardiac function gradeⅢ,and futher decreased in patients with cardiac function gradeⅢthan those in patients with cardiac function gradeⅣ.LVEF in patients with cardiac function gradeⅠ-Ⅱwas greater than in patients with cardiac function gradeⅢ,and in patients with cardiac function gradeⅢwas greater than that in patients with cardiac function gradeⅣ(P<0.05).The LVEDV and LVESV were positively correlated with cardiac function grading(r=0.583,0.734,P<0.01),while LVEF was negatively correlated with cardiac function grading(r=-0.782,P<0.01).Conclusion It is demonstrated that MRI parameters were significantly correlated with NYHA cardiac function grading of DCM patients,which could effectively evaluate condition of DCM.
