摘要
目的:研究不同血压维持水平在神经纤维瘤合并脊柱侧弯患者术中护理中的应用价值。方法:将某院收治的70例神经纤维瘤合并脊柱侧弯患者随机分为对照组和观察组各35例,术中对照组患者将血压指标MAP控制性维持在(70±5)mmHg水平,观察组控制性维持在(60±5)mmHg水平。于两组患者手术前后,分别检查其血清应激刺激反应指标(血清NA、E、COR)、动脉血气指标(PaO_(2)、PaCO_(2))和脊髓功能指标(腓总神经与胫总神经DL、NCV)。结果:术后,观察组患者血清NA、E、COR均低于对照组(P<0.05);术中,观察组患者PaCO_(2)低于对照组(P<0.05),观察组患者PaO_(2)高于对照组(P<0.05)。观察组患者腓总神经与胫总神经DL低于对照组(P<0.05),观察组患者腓总神经与胫总神经NCV均高于对照组(P<0.05)。结论:于神经纤维瘤合并脊柱侧弯患者术中将MAP控制性维持在(60±5)mmHg水平可有效降低手术应激刺激,促进动脉血气的稳定,有利于对患者脊髓的保护,进而提高脊髓功能。
Objective:To study the application value of different blood pressure maintenance levels in the intraoperative nursing of patients with neurofibroma complicated with scoliosis.Methods:A total of 70 patients with neurofibroma complicated with scoliosis admitted to a hospital were randomly divided into control group(n=35)and observation group(n=35).During the operation,MAP of blood pressure index was maintained at(70±5)mmHg in control group and(60±5)mmHg in observation group.Before and after operation,stress response indexes(serum NA,E,COR),arterial blood gas indexes(PaO_(2),PaCO_(2))and spinal cord function indexes(peroneal,tibial nerve DL,NCV)were examined.Results:After operation,serum NA,E and COR in observation group were lower than those in control group(P<0.05).During operation,PaCO_(2)in observation group was lower than that in control group(P<0.05),and PaO_(2)was higher than that in control group(P<0.05).The total peroneal and tibial nerve DL in observation group was lower than that in control group(P<0.05),and NCV was higher than that in control group(P<0.05).Conclusion:In patients with neurofibroma complicated with scoliosis,MAP control at the level of(60±5)mmHg can effectively reduce surgical stress,promote the stability of arterial blood gas,and facilitate the protection of spinal cord,thus improve spinal cord function.
