摘要
目的探讨催眠疗法对高龄剖宫产初产妇焦虑情绪的影响。方法选取2018年6月至2019年9月濮阳市妇幼保健院收治的87例伴焦虑情绪的高龄初产妇的临床资料进行回顾性分析。产妇均接受剖宫产分娩。根据干预方案将产妇分为对照组(43例)和研究组(44例)。给予对照组常规产前干预措施,研究组于对照组基础上增设针对性催眠疗法。分别于入院时、产前1h采用汉密顿焦虑量表(HAMA)、一般自我效能感量表(GSES)评估产妇的焦虑情绪、自我效能感。比较两组入院时、产前1h心血管应激反应指标,包括平均动脉压(MAP)、心率(HR)。统计并比较两组术中寒战发生情况、护理满意度和术后住院时间。结果产前1h,两组HAMA评分均低于入院时,研究组HAMA评分较对照组低(P<0.05)。产前1h,两组GSES评分高于入院时,研究组GSES评分较对照组高(P<0.05)。产前1h,两组MAP、HR均较入院时升高,但研究组MAP、HR较对照组低(P<0.05)。研究组术中寒战发生率较对照组低,护理满意度较对照组高,术后住院时间较对照组短(P<0.05)。结论在剖宫产前,对伴焦虑情绪的高龄初产妇施行针对性催眠疗法,能显著改善产妇的自我效能,减轻焦虑情绪,降低心血管应激反应,减少术中寒战发生,提高护理满意度,促进术后恢复。
Objective To explore the effect of hypnotherapy on the anxiety of senile primipara during cesarean section.Methods The clinical data of 87 elderly primiparas with anxiety from June 2018 to September 2019 in Puyang Maternal and Child Health Hospital were selected for retrospective analysis.All of them underwent cesarean section delivery.They were divided into control group(43 cases)and study group(44 cases).The control group was given routine prenatal interventions,and the study group was given targeted hypnot herapy on the basis of control group.Hamilton anxiety scale(HAMA)and general self-efficacy scale(GSES)were used to evaluate maternal anxiety and self-efficacy at the time of admission and one hour before delivery.The cardiovascular stress indexes,incuding mean arterial pressure(MAP)and heart rate(HR),were compared between the two groups at the time of admission and one hour before delivery.The incidence of intraoperative chills,nursing satisfaction and postoperative hospitalizat ion time were statistically compared between the two groups.Results One hour before delivery,the HAMA scores of both groups were lower than those at admission,and the HAMA score of study group was lower than that of control group(P<0.05).One hour before delivery,the GSES scores of the two groups were higher than those at admission,and the GSES score of study group was higher than that of control group(P<0.05).At 1 hour before delivery,the MAP and HR of both groups were higher than those at admission,and MAP and HR of study group were lower than those of control group(P<0.05).The incidence of shivering in study group was lower than that in control group,and the nursing satisfaction was higher than that in control group,and the postoperative hospitalization time was shorter than that in control group(P<0.05).Conclusion Before cesarean section,targeted hypnotherapy for elderly primipara with anxiety can significantly improve the self-effcacy of parturient,relieve anxiety,reduce cardiovascular stress,reduce the occurrence of intraoperative chills,improve nursing satisfaction and promnote postoperative recovery.
