摘要
目的 调查分析某儿童医院2020年门急诊不合理处方情况,促进临床合理用药,保障用药安全。方法 采用回顾性分析方法,随机抽取2020年门急诊处方共12462张进行点评,对不合理处方用帕累托法分析原因。结果 合理处方12272张,合理率98.47%。不合理处方190张,不合理率1.52%。帕累托法分析显示,主要不合理处方类型是无适应症用药(146张,76.44%),次要不合理处方类型是适应症不适宜(15张,7.85%),一般不合理类型为重复用药(13张,6.81%),用法用量不适宜(10张,5.24%),不规范处方(3张,1.57%),超说明书用药(3张,1.57%)和联合用药不适宜(1张,0.52%)。结论 帕累托分析法可直观表现处方不合理的主要类型,针对性提出改进方案,有效提高处方质量和用药合理性。
OBJECTIVE To improve the rationality and security of clinical medication,the unreasonable prescriptions of outpatient clinics and emergency department at a children′s hospital in 2020 were investigated and analyzed.METHODS A total of 12,462 outpatient and emergency clinic prescriptions in 2020 were randomly selected for review by retrospective analysis,the reasons for unreasonable prescriptions were analyzed by using the Pareto method.RESULTS There were 12,272 reasonable prescriptions,accounting for 98.47%,and 190 unreasonable prescriptions,accounting for 1.52%.Pareto analysis showed that the primary type was no indications(n=146,76.44%),secondary type was unsuitable indications(n=15,7.85%),general unreasonable types were duplicated medications(n=13,6.81%),improper usage and dosage of administration(n=10,5.24%),non-standard prescription(n=3,1.57%),off-label use(n=3,1.57%) and improper drug combinations(n=1,0.52%).CONCLUSION Pareto analysis can directly show the main types of irrational prescriptions,put forward targeted improvement schemes,and effectively improve the quality of prescriptions and rationality of medication.
