摘要
幽门螺杆菌(Hp)是引发胃癌最重要且可控的危险因素,根除Hp可有效降低胃癌的发生风险。当前根除Hp的主流方案是联合应用2~3种抗生素,疗程为10或14 d,由于抗生素耐药日益严峻及胃肠道不良反应的出现,Hp的根除往往难以达到预期效果。儿童和老年人是Hp感染的特殊人群,在Hp根除治疗中存在着一定的局限性和特殊性,因此需要寻求毒副作用少、根除率高、不良反应发生率低的根除方法。益生菌具有改善胃肠道微生态环境、减轻炎症反应等作用,在提高Hp根除率及减少药物不良反应方面具有显著优势,联合微生态疗法治疗Hp感染有望成为治疗特殊人群Hp感染的新路径。
Helicobacter pylori(Hp)is the most important and controllable risk factor for gastric cancer,and eradication of Hp can effectively reduce the risk of gastric cancer.The current mainstream program for Hp eradication is the combined use of 2 to 3 antibiotics for 10 or 14 days.Due to the increasingly severe antibiotic resistance and the emergence of gastrointestinal adverse reactions,the eradication of Hp is often difficult to achieve the expected effect.Children and the elderly are special populations of Hp infection,and there are certain limitations and particularities in their Hp eradication therapy.Therefore,it is necessary to seek the eradication methods with few toxic side effects,high eradication rate,and low incidence of adverse reactions.Probiotics can improve the microecological environment of the gastrointestinal tract,reduce inflammation,etc.,which have significant advantages in improving the eradication rate of Hp and reducing adverse drug reactions.Combining microecological therapy for Hp infection is expected to become a new way to treat Hp infection in the special populations.
