摘要
目的在经胸超声心动图(TTE)中,为减少较小房间隔缺损(ASD)或卵圆孔未闭(PFO)的漏诊,探讨Valsalva动作在TTE中剑下双房切面的显示。方法前瞻性随机研究深圳市龙岗区第二人民医院超声科2020年1月至2021年9月201例行TTE检查者,常规超声显示剑下双房切面和常规超声结合Valsalva动作显示剑下双房切面,比较两种方法的显示。对常规超声双房切面不显示者,分析Valsalva动作在TTE中剑下双房切面显示是否与脂肪肝、中晚期妊娠相关。结果201例受检者中,常规超声显示137例(68.16%),常规超声结合Valsalva动作显示182例(90.55%),差异具有统计学意义(χ^(2)=30.764,P<0.05)。Valsalva动作在TTE中剑下双房切面显示与脂肪肝和中晚期妊娠相关,列联系数rn分别为0.294、0.493(P<0.05)。结论常规超声结合Valsalva动作能提高TTE中剑下双房切面显示率,对于基层医院具有非常重要应用价值。脂肪肝、中晚期妊娠可影响Valsalva动作在TTE中剑下双房切面显示。
Objective To investigate the effect of Valsalva maneuver on the display of subxiphoid biatrial views in transthoracic echocardiography(TTE),so as to reduce the missed diagnosis of small atrial septal defects(ASD)or patent foramen ovale(PFO).Methods A total of 201 subjects undergoing TTE in the Department of Ultrasound of the Second People’s Hospital of Longgang District in Shenzhen from January 2020 to September 2021 were prospectively and randomly studied.The subxiphoid biatrial views were displayed with conventional ultrasound,and with conventional ultrasound+Valsalva maneuver,respectively.The displays under both methods were compared.For those subxiphoid biatrial views did not show with conventional ultrasound,the correlation between Valsalva maneuver and fatty liver and medium-late pregnancy in the display of subxiphoid biatrial views in TTE was analyzed.Results In 201 subjects,137(68.16%)subxiphoid biatrial views were displayed with conventional ultrasound,and 182(90.55%)were displayed with conventional ultrasound+Valsalva maneuver,with statistically significant difference(χ^(2)=30.764,P<0.05).There was a correlation between Valsalva maneuver and fatty liver and medium-late pregnancy in the display of subxiphoid biatrial views in TTE,with contingency coefficient rn of 0.294 and 0.493(P<0.05),respectively.Conclusion Conventional ultrasound+Valsalva maneuver can improve the display rate of subxiphoid biatrial views in TTE,which has significant application value in grass-roots hospitals.In addition,it is found that fatty liver and medium-late pregnancy can affect the display of subxiphoid biat rial views in TTE with the Valsalva maneuver.
