摘要
目的:建立重组人血小板生成素(recombinant human thrombopoietin, Rh-TPO)合理性使用评价标准,为临床上提高Rh-TPO的合理性使用提供参考依据。方法:以Rh-TPO的说明书为基础,参照相关专家共识及循证医学证据制定合理性评价标准,采用加权TOPSIS法对甘肃省人民医院2020年出院病历进行合理性评价。结果:使用Rh-TPO的186例病历中,用药合理的患者25例(13.44%),基本合理的患者151例(81.19%),用药不合理的患者10例(5.38%)。结论:加权TOPSIS法可用于Rh-TPO合理性评价,结果表明该院Rh-TPO临床应用过程中基本合理,但存在给药剂量不适宜、给药疗程较短、给药时机不适宜等不合理现象,需加大管理力度,以保证临床合理使用Rh-TPO。
OBJECTIVE The evaluation criteria for the rational use of recombinant human thrombopoietin(Rh-TPO)is established, in order to provide a reference for improving the rational use of Rh-TPO in clinical practice.METHODS Based on the Rh-TPO instructions, the rationality evaluation criteria were formulated with reference to the consensus of relevant experts and evidence-based medical evidence, and the weighted TOPSIS method was used to evaluate the rationality of the discharge medical records of Gansu Provincial People′s Hospital in 2020.RESULTS Among the 186 medical records using Rh-TPO,the ratio of patients with reasonable medication was 13.44%(25/186),the ratio of patients with basically reasonable medication was 81.19%(151/186),and the ratio of patients with irrational medication was 5.38%(10/186).CONCLUSION The weighted TOPSIS method can be used to evaluate the rationality of Rh-TPO.The results showed that the clinical application of Rh-TPO in this hospital was basically reasonable, but there were still some unreasonable uses of Rh-TPO,for example, the existence of inappropriate dosage, short course of treatment, and inappropriate timing of administration.It is necessary to strengthen management to ensure safe use of Rh-TPO in clinic.
