摘要
目的对高血压患者DRG分组效果进行评价并分析住院费用影响因素,为DRG付费提供参考。方法收集某三甲医院高血压患者住院信息,采用变异系数和总方差减少系数方法评价高血压DRG分组效果,利用步进法多元线性回归分析住院费用的影响因素。结果DRG分组组内一致性较好,组间异质性不够明显;住院天数、合并症数量、耗材费占比、有无手术操作、年龄以及药费占比对住院费用具有一定的影响。结论DRG分组能够客观反映高血压疾病医疗资源消耗水平,可以为医保付费提供参考;缩短住院天数,合理用药可以有效降低住院费用,控制医保支出费用。
Objective To evaluate the grouping effect of DRG and to explore the factors affecting hospitalization expenses in hypertensive patients,so as to provide reference for improving DRG payment.Methods The hospitalization information of hypertensive patients in a tertiary hospital was collected,and the grouping effect of DRG in hypertensive patients was evaluated using coefficient of variation and total variance reduction.The influencing factors of hospitalization expenses were analyzed by the stepwise multiple linear regression.Results The intra-group consistency of DRG was good,but the heterogeneity between groups was not obvious.The length of stay,the number of complications,the proportion of consumable fee,surgery or not,age,and the proportion of medicine fee were the influencing factors of hospitalization expenses.Conclusion DRG could objectively reflect the level of medical resource consumption in hypertensive patients which provides reference for medical insurance payment.It can also shorten the length of stay,promote rational drug use,effectively reduce hospitalization expenses and control medical insurance payment.
