Short-term efficacy of proprioceptive neuromuscular stimulation combined with repetitive transcranial magnetic stimulation in improving upper limb function of stroke patients with hemiplegia

摘要 目的 观察本体感觉神经肌肉促进术(PNF)结合重复性经颅磁刺激(rTMS)治疗改善脑卒中偏瘫患者上肢功能的短期疗效。方法 60例首次脑卒中偏瘫患者随机均分为三组,均接受常规康复治疗。A组加用PNF治疗;B组加用rTMS治疗;C组加用PNF和rTMS治疗。治疗3周后评估疗效,比较三组患者治疗前后运动功能状态量表(MSS)评分、偏瘫上肢功能评定量表(香港版)(FTHUE-HK)评分、Barthel指数(BI)评分和受累侧运动诱发电位(MEP)波幅最大峰值。结果 三组患者治疗后的MSS、FTHUE-HK、BI评分和受累侧MEP波幅最大峰值均较治疗前改善(P<0.05)。C组患者MSS评分、FTHUE-HK评分和受累侧MEP波幅最大峰值改善程度均优于A组和B组(P<0.05),但BI评分改善程度仅优于B组(P<0.05)。结论 在常规康复治疗基础上,加用PNF或rTMS短期治疗均能改善脑卒中偏瘫患者上肢功能,联合采用PNF和rTMS的疗效更好。 Objective To observe the short-term efficacy of proprioceptive neuromuscular stimulation(PNF) combined with repetitive transcranial magnetic stimulation(rTMS) in improving the upper limb function of stroke patients with hemiplegia.Methods Sixty patients with first stroke hemiplegia were randomly divided into three groups with 20 cases each.On the basis of conventional rehabilitation therapy, group A was additionally given PNF treatment, group B was additionally use of rTMS treatment, and group C was additionally given PNF and rTMS.The short-term efficacy was evaluated by the motor function status scale(MSS) score, the functional test of the hemiplegic upper extremity-Hong Kong(FTHUE-HK) score and the Barthel index(BI) score, and the maximum peak amplitude of the affected side motor evoked potential(MEP) after treatment for three weeks, which were compared among the three groups.Results Compared to before, the scores of MSS,FTHUE-HK and BI,and the maximum peak amplitude of MEP on the affected side of three groups were all improved obviously after treatment for three weeks(P<0.05).The improvements in the MSS score, and FTHUE-HK score and maximum peak amplitude of MEP on the affected side were better in group C than those in groups of A and B(P<0.05).The improvement of BI score was better in group C than that in group B(P<0.05).Conclusion On the basis of conventional rehabilitation therapy, additionally short-term use of PNF or rTMS treatment, especially combined use of PNF and rTMS,can effectively improve the upper limb function of stroke patients with hemiplegia.
