miR-125b及miR-210在AECOPD患者血清中的表达水平及作用机制分析 被引量:1

摘要 目的:研究miR-125b及miR-210在慢性阻塞性肺疾病急性加重期(AECOPD)患者血清中的表达水平及作用机制。方法:将2018年12月1日-2020年12月10日沈阳市苏家屯区中西医结合医院收治的113例COPD患者纳入研究,其中包括60例AECOPD患者(AECOPD组),53例COPD稳定期患者(COPD稳定期组)。另取同期于医院进行体检健康人员50例作为对照组。以肺功能仪对所有受试者肺功能进行检测并对比,通过实时荧光定量聚合酶链反应(qRT-PCR)对所有受试者血清miR-125b及miR-210水平进行检测并对比。以Pearson相关性分析血清miR-125b、miR-210水平和肺功能指标的关系。此外,以受试者工作特征(ROC)曲线分析血清miR-125b、miR-210水平诊断AECOPD的效能。结果:AECOPD组及COPD稳定期组用力肺活量(FVC)、第1秒用力呼气量占用力肺活量比值(FEV;/FVC%)、呼气峰值流速(PEF)水平均低于对照组,且AECOPD组上述各项指标均低于COPD稳定期组(P<0.05)。AECOPD组、COPD稳定期组及对照组的血清miR-125b水平呈逐渐降低趋势,而miR-210水平呈逐渐升高趋势,且经单因素方差分析发现各组间对比差异均有统计学意义(P<0.05)。经Pearson相关性分析可得:血清miR-125b和FVC、FEV;/FVC%、PEF水平均呈负相关,而miR-210和FVC、FEV;/FVC%、PEF水平均呈正相关关系(P<0.05)。经ROC曲线分析发现:miR-125b联合miR-210诊断AECOPD的曲线下面积、灵敏度、特异度及约登指数均高于上述两项指标单独诊断(P<0.05)。结论:miR-125b在AECOPD患者中异常高表达,而miR-210在AECOPD患者中异常低表达,两者联合检测可能有利于诊断AECOPD。 Objective:To investigate the serum expression levels and mechanisms of miR-125b and miR-210 in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Method:A total of 113 patients with COPD admitted to Sujiatun District Hospital of Integrated Traditional Chinese and Western Medicine of Shenyang City from December 1,2018 to December 10,2020 were included in this study,including 60 patients with AECOPD (AECOPD group) and 53 patients with stable COPD (COPD stable stage group).Another 50 healthy people who underwent physical examination in hospital during the same period were selected as the control group.Lung function of all subjects was detected and compared by pulmonary function analyzer,serum miR-125b and miR-210 levels of all subjects were detected and compared by real-time fluorescence quantitative polymerase chain reaction (qRT-PCR).The relationship between serum miR-125b and miR-210 levels and pulmonary function indexes were analyzed by Pearson correlation.In addition,receiver operating characteristic (ROC) curve was used to analyze the diagnostic efficacy of serum miR-125b and miR-210 levels in AECOPD.Result:The levels of FVC,FEV;/FVC% and PEF in AECOPD group and COPD stable stage group were lower than those in the control group,and the above indexes in AECOPD group were lower than those in COPD stable stage group (P<0.05).The serum miR-125b levels in AECOPD group,COPD stable stage group and control group showed a gradually decreasing trend,while the serum miR-210 levels showed a gradually increasing trend,and the differences among all groups were statistically significant by one-way analysis of variance (P<0.05).According to Pearson correlation analysis,serum miR-125b were negatively correlated with FVC,FEV;/FVC% and PEF levels,while miR-210 were positively correlated with FVC,FEV;/FVC% and PEF levels (P<0.05).ROC curve analysis showed that the area under the curve,sensitivity,specificity and Youden index of miR-125b combined with miR-210 in the diagnosis of AECOPD were higher than those of the above two indicators in the diagnosis of AECOPD alone (P<0.05).Conclusion:The abnormally high expression of miR-125b and abnormally low expression of miR-210 in AECOPD patients may be beneficial for the diagnosis of AECOPD.
