摘要
目的研究角膜荧光素钠染色对Pentacam检查的影响。方法前瞻性临床研究。随机将100名志愿者分成实验组和对照组,各50名(100只眼),实验组结膜囊内滴入0.1%的荧光色钠,对照组滴入0.9%生理盐水,分别对滴入前、后角膜进行Pentacam检查,采集各组不同时间点观察指标的数据,采用重复测量方差分析。结果中央角膜厚度(CCT)时间效应与组间效应存在交互作用,差异具有统计学意义(F=220.83,P<0.05)。从时间效应看,实验组受时间影响,差异有统计学意义(F=469.42,P<0.001),CCT在刚滴入后的值最高(573.36±44.53)μm,且随时间呈下降趋势,于滴入后20 min趋于滴入前CCT水平。从组间效应看,刚滴入时、滴入后5 min、滴入后10 min两组差异均具有统计学意义(P<0.05),且实验组均值较对照组明显增加。前表面角膜散光(AA)(F=2.73,P<0.05),组间效应中只有刚滴入时(F=5.35,P=0.02)两组差异具有统计学意义,且实验组AA均值较对照组高;而角膜前表面曲率(ASK)、角膜后表面曲率(PSK)、后表面散光(PA)、前房深度(ACD)、角膜表面高度指标,无论是时间效应还是组间效应差异均无统计学意义(P>0.05)。结论荧光素钠染色会使CCT、AA的Pentacam测量数据不同程度增加,且受时间影响,而对ASK、PSK、PA、ACD、角膜表面高度的测量无影响。
Objective To investigate the effect of corneal fluorescein sodium staining on Pentacam examination.Methods Prospective clinical study.One hundred volunteers were randomly divided into the experimental group and the control group,each with 50 volunteers(100 eyes).Participants in the experimental group were instilled with 0.1%fluorescent sodium in the conjunctival sac,and participants in the control group were instilled with 0.9%saline.The corneas were examined by Pentacam before and after the procedure.The data of observation indexes at different time points in each group were collected,using repeated measures ANOVA.Results The time effect of central corneal thickness(CCT)had an interaction with the between group effect,and the difference was statistically significant(F=220.83,P<0.05).Regarding the time effect,the experimental group was affected by the time,and the difference was statistically significant(F=469.42,P<0.001).The value of CCT was the highest immediately after instillation(573.36±44.53),and decreased over time.The CCT value tended to return to the pre-instillation level at 20 minutes after the instillation.Regarding the between-group effect,there were significant differences between the two groups at the beginning of instillation,5 minutes after instillation and 10 minutes after instillation(P<0.05),and the mean value in the experimental group was significantly higher than that in the control group.Significant between-group difference was observed for the anterior surface corneal astigmatism(AA)(F=2.73,P<0.05)only at the time of instillation(F=5.35,P=0.02),and the mean AA in the experimental group was higher than that in the control group;while no significant between-group difference was observed for the anterior corneal surface curvature(ASK),posterior corneal surface curvature(PSK),posterior surface astigmatism(PA),anterior chamber depth(ACD)or corneal surface height index,regardless of the time effect or the between group effect(P>0.05).Conclusions Sodium fluorescein staining can increase the CCT and AA value measured by pentacam in varying degrees,and is affected by time,but has no effect on the measurement of ASK,PSK,PA,ACD,and corneal surface height.
