摘要
目的探讨下肢静脉曲张应用超声下大隐静脉高位结扎加微波消融联合泡沫硬化剂治疗的临床疗效。方法选择2019年6月—2020年12月期间来该院治疗的下肢静脉曲张患者120例作为研究对象,随机分为参考组(常规治疗,n=60)和分析组(超声下大隐静脉高位结扎加微波消融联合泡沫硬化剂治疗,n=60),对比手术相关指标、症状改善总有效率、生活质量评分、并发症发生率。结果分析组手术时间(50.65±7.44)min,术中出血量(21.54±4.87)mL,皮下淤血范围(2.03±0.87)cm,住院时间(4.28±1.11)d,切口疼痛评分(1.32±0.41)分均低于参考组,差异有统计学意义(t=23.552、56.264、18.436、10.606,16.107,P<0.05);症状改善总有效率98.33%高于参考组83.33%,差异有统计学意义(χ^(2)=8.106,P<0.05);治疗后总体健康(93.65±2.54)分,精神状态(96.47±2.01)分,心理功能(92.88±5.87)分,生理功能(93.04±2.48)分高于参考组,差异有统计学意义(t=17.030、26.161、8.843、16.399,P<0.05),并发症发生率1.67%低于参考组15.00%,差异有统计学意义(χ^(2)=6.981,P<0.05)。结论通过应用超声下大隐静脉高位结扎加微波消融与泡沫硬化剂联合治疗的方式可以促进下肢静脉曲张患者诊疗效果的提升,适合推广使用。
Objective To investigate the clinical efficacy of ultrasonic high ligation of great saphenous vein plus microwave ablation combined with foam sclerotherapy in the treatment of varicose veins of the lower extremities.Methods 120 patients with varicose veins of the lower extremities who came to the hospital for treatment from June 2019 to December 2020 were selected as the research objects.They were randomly divided into the reference group(conventional treatment,n=60)and the analysis group(high ligation of the great saphenous vein under ultrasound plus microwave ablation combined with foam sclerotherapy,n=60).Compared operation related indexes,total effective rate of symptom improvement,quality of life score and incidence of complications.Results Analysis group operation time(50.65±7.44)min,intraoperative blood loss(21.54±4.87)mL,subcutaneous congestion range(2.03±0.87)cm,hospital stay(4.28±1.11)d,incision pain score(1.32±0.41)points.were lower than the reference group,the difference was statistically significant(t=23.552,56.264,18.436,10.606,16.107,P<0.05).The total effective rate of symptom improvement was 98.33%higher than that of the reference group 83.33%,the difference was statistically significant(χ^(2)=8.106,P<0.05).After treatment,overall health(93.65±2.54)points,mental state(96.47±2.01)points,mental function(92.88±5.87)points,and physiological function(93.04±2.48)points were higher than the reference group,the difference was statistically significant(t=17.030,26.161,8.843,16.399,P<0.05).The complication rate was 1.67%lower than the reference group 15.00%,the difference was statistically significant(χ^(2)=6.981,P<0.05).Conclusion The combination of high ligation of the great saphenous vein under ultrasound,microwave ablation and foam sclerotherapy can improve the diagnosis and treatment of varicose veins in the lower extremities,which is suitable for popularization.
