摘要
目的:分析比较不同医保类型冠心病患者住院费用及其影响因素,为控制医疗费用的不合理增长提供建议。方法:以山东省某三甲医院2018年冠心病患者住院费用为研究对象,应用描述性分析,秩和检验和多重线性回归分析比较不同医保类型患者的住院费用构成及影响因素。结果:共纳入6034例患者,职工医保、居民医保和自费患者分别占比47.48%、41.10%和11.42%。住院费用构成中,各类医保患者的卫生材料费、西药费和治疗费占比较高。除了西药费,其他费用各组间差异均有统计学意义(P<0.05)。结论:不同医保类型冠心病患者的住院费用构成存在差异,卫生材料费和西药费对医疗总费用起着主导作用。性别、年龄、来源地、医保类型、住院天数以及是否手术是冠心病患者的住院费用的主要影响因素。
Objective:To analyze the hospitalization expenses and influencing factors for patients with coronary heart disease under different types of basic medical insurances,so as to provide a basis for effectively controlling of the unreasonable growth of the medical expenses.Methods:Taking the hospitalization expenses of patients with coronary heart disease in a grade-A tertiary hospital in Shandong in 2018 as the research object,the descriptive analysis,rank sum test and multiple linear regression analysis were used to analyze hospitalization expenses component and its influencing factors.Results:A total of 6034 patients were enrolled.Basic medical insurance scheme for employees,basic medical insurance scheme for urban and rural residents,and patients at their own expense accounted for 47.48%,41.10%and 11.42%,respectively.Among the various hospitalization expenses,health material expenses and western medicine expenses in all types of insured patients were relatively high.Except for western medicine expenses,there were statistically significant differences in other groups of expenses(P<0.05).Conclusion:There are significant differences in the hospitalization expenses for patients with coronary heart disease under different types of basic medical insurances,health material expenses and western medicine expenses were the main components.Gender,age,habitation,medical insurance type,hospitalization days and surgery or not were the main influencing factors of the hospitalization expenses.
