辅助应用特布他林雾化吸入治疗方案对肺炎患儿血清炎性因子及辅助性T淋巴细胞亚群水平的影响

Effects of adjuvant terbutaline aerosol inhalation on serum inflammatory factors and helper T lymphocyte subsets in children with pneumonia

摘要 目的探讨辅助应用特布他林雾化吸入治疗方案对肺炎患儿血清炎性因子及辅助性T淋巴细胞亚群水平的影响。方法选取张家港市第二人民医院2018年11月—2020年11月收治的84例肺炎患儿,采用随机数字表法将其分成对照组和观察组,各42例。对照组采用常规治疗,观察组在此基础上辅助应用特布他林雾化吸入治疗方案,比较治疗前后2组患儿血清炎性因子[白介素(IL)-1、IL-6、肿瘤坏死因子α(TNF-α)、C反应蛋白(CRP)、降钙素原(PCT)]、辅助性T淋巴细胞亚群(Th1、Th2、Th1/Th2、CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)、CD_(4)^(+)/CD_(8)^(+))、临床疗效以及不良反应发生情况。结果治疗后,2组IL-1、IL-6、TNF-α、CRP、PCT低于治疗前,且观察组IL-1、IL-6、TNF-α、CRP、PCT低于对照组(P<0.05)。治疗后,2组Th1、Th1/Th2、CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)高于治疗前,Th2、CD_(8)^(+)低于治疗前,且观察组Th1、Th1/Th2、CD_(3)^(+)、CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)高于对照组,Th2、CD_(8)^(+)低于对照组(P<0.05)。治疗后观察组总有效率为95.24%,高于对照组的76.19%(χ^(2)=6.222,P=0.013)。2组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论辅助应用特布他林雾化吸入治疗方案治疗肺炎患儿疗效明显,可有效调节辅助性T淋巴细胞亚群水平,改善免疫功能,抑制炎性反应,安全性较高。 Objective To investigate the effects of terbutaline aerosol inhalation on serum inflammatory factors and helper T lymphocyte subsets in children with pneumonia.Methods A total 84 children with pneumonia who admitted to the were selected from November 2018 to November 2020 in Zhangjiagang Second People's Hospital for the study subjects.All the children were divided into control group and observation group by random number table method.42 cases in each group.Control group was treated with conventional treatment,and the observation group was treated with terbutaline aerosol inhalation on the basis of the control group.Serum levels of inflammatory cytokines(IL-1,IL-6,TNF-α,CRP,PCT),helper T lymphocyte subsets(Th1,Th1/Th2,CD_(3)^(+),CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+)),the curative effect and the incidence of adverse reactions were compared between the two groups.Results After treatment,IL-1,IL-6,TNF-α,CRP and PCT in the both group were lower than those before treatment(P<0.05).After treatment,IL-1,IL-6,TNF-α,CRP and PCT in the observation group were lower than those in the control group(P<0.05).After treatment,Th1,Th2,Th1/Th2,CD_(3)^(+),CD_(4)^(+),CD_(8)^(+),CD_(4)^(+)/CD_(8)^(+) in the both group were higher than those before treatment,and Th2,CD_(8)^(+)in the both group were lower than those before treatment(P<0.05).After treatment,Th1,Th1/Th2,CD_(3)^(+),CD_(4)^(+)and CD_(4)^(+)/CD_(8)^(+) in the observation group were higher than those in the control group,and Th2 and CD_(8)^(+)in the observation group were lower than those in the control group(P<0.05).The total effective rate of the observation group(95.24%)was higher than that of the control group(76.19%)(χ^(2)=6.222,P=0.013).There was no significant difference in the incidence of adverse reactions between the observation group and the control group(P>0.05).Conclusion Terbutaline aerosol inhalation program has significant efficacy in the treatment of children with pneumonia,which can effectively regulate helper T lymphatic subsets,inhibit inflammatory response,improve immune function,and has high safety.
