摘要
目的探讨以需求为导向的健康教育在慢性阻塞性肺气肿(chronic obstructive pulmonary emphysema,COPE)患者中的应用效果。方法选取2018年7月至2020年3月江苏省淮安市第一人民医院收治的COPE患者86例,采用随机数字表法分为试验组44例、对照组42例,两组均给予相同的治疗及护理干预、健康教育。对照组采用常规健康教育,试验组联合应用以需求为导向的健康教育。随访6个月,比较两组疾病认知水平、自我管理能力。结果试验组患者COPE基础知识、自我护理知识、肺康复训练知识、疾病认知水平总分高于对照组(t=3.855、3.774、5.233、4.909,P均<0.05)。症状管理、日常生活管理、情绪管理、信息管理、自我效能管理、自我管理能力总分高于对照组(t=3.794、4.207、6.602、3.244、4.473、4.231,P均<0.05)。患者满意度95.45%高于对照组80.95%(χ^(2)=4.398,P<0.05)。结论以需求为导向的健康教育更容易被COPE患者所认同与接受,有助于提高其疾病认知水平,促进自我管理能力的养成。
Objective To evaluate the effect of demand-oriented health education on the disease cognition level and selfmanagement skills in patients with chronic obstructive pulmonary emphysema(COPE).Method A total of 86 COPE patients in Huai’an First People’s Hospital from July 2018 to March 2020 were enrolled in this study.Patients were divided into educational groups(n=44)and control group(n=42)by using random number table.Both groups provided the same treatment,nursing and health education contents.Control group received the regular health education while the educational group provided demand-oriented health education.Disease cognition level and self-management skills were compared at 6-month follow-up.Result The total score of basic COPE knowledge,self-nursing knowledge,pulmonary rehabilitation training knowledge,disease cognition level in educational group were higher than control group(t=3.855,3.744,5.233,4.909,P<0.05).The total score of symptom management,daily life management,emotion management,information management,self-efficacy management,and self-management skills educational group were higher than control group(t=3.794,4.207,6.602,3.244,4.473,4.231,P<0.05).In addition,the satisfaction educational group was also significantly higher than control group(95.45% vs.80.95% χ^(2)=4.398,P <0.05).Conclusion Demand-oriented health education is easily recognized and accepted by COPE patients.It is helpful for improvement of their disease cognition level and their selfmanagement skills.
