摘要
目的探讨基于薄层CT引导的气管镜刷检在肺外周病灶诊断中的价值。方法回顾性分析56例薄层CT提示肺外周病灶的患者,治疗前通过薄层CT引导的气管镜检查,细胞学标本送检病理科,计算基于薄层CT引导的气管镜刷检在肺外周病灶的诊断效能。结果薄层CT引导的气管镜刷检诊断阳性29例,阳性率51.79%,灵敏度60.42%。该方法诊断肺外周病灶的阳性率与病灶性质、病灶直径、病灶与支气管的关系有关(P<0.05)。结论对于倾向恶性的肺外周病灶,基于薄层CT引导的气管镜检查是一种非常经济有效的诊断方法,适合在没有开展电磁导航等高级支气管镜技术的医院推广。
Objective To evaluate the value of bronchoscope brushing guided by thin-layer CT in the diagnosis of peripheral pulmonary lesions.Methods A retrospective analysis of 56 patients with thin-slice CT suggesting peripheral lung lesions was performed.Before treatment,the patients were undergoing a thin-slice CT-guided bronchoscopy,and the cytology specimens were sent to the pathology department to calculate the diagnostic efficiency of the thin-slice CT-guided bronchoscopy in the peripheral lung lesions.Results The thin-slice CT-guided bronchoscopy had a positive diagnosis in 29 cases,with a positive rate of 51.79%and a sensitivity of 60.42%.The positive rate of this method in the diagnosis of peripheral lung lesions was related to the nature of the lesion,the diameter of the lesion,and the relationship between the lesion and the bronchus(P<0.05).Conclusion For peripheral lung lesions that tend to be malignant,thin-layer CT-guided bronchoscopy is a very economical and effective diagnostic method,which is suitable for promotion in hospitals that do not have advanced bronchoscopy techniques such as electromagnetic navigation.
