摘要
住院医师规范化培训于2015年在我国全面铺开,是临床医学毕业生走向岗位前继续接受医学教育的重要组成部分,是年轻医师成长的必由之路。内科学是是临床各科的基础。而风湿免疫科是内科学的一个重要分支,是内科学轮转中的必修科室。风湿病临床表现多样,内容抽象,易与其他学科交叉,是住院医师规范化培训中难度较高的科室之一。现总结科室多年来的教学经验,包括以下几个方面:(1)完善入科教育,掌握风湿科基础知识;(2)采用临床路径法,LBL(lecture-based learning)、CBL(case-based learning)及PBL(problem-based learning)三轨结合以及运用微课翻转课堂教学模式等多种模式,丰富临床教学,掌握常见风湿病诊疗;(3)增加实操体验,重视临床实践技能的培养;(4)通过疑难病历教学查房,提高规培医师的整体临床思维;(5)“每周一讲”培养性讲课,提高规培生自主学习能力。通过以上多个方面教学,运用多种教学手段,引导规培生掌握培训内容要求,逐步形成良好的整体临床思维,发挥自主学习能力,提高风湿免疫科的规培教学质量,从而提升规培生的综合能力,成长为合格的临床医师。
Standardized training for resident physicians was fully launched in China in 2015,which is an important part for clinical medicine graduates to continue to receive medical education before entering the post and is the only way for young doctors to grow up.Internal medicine is the basis of the training.Rheumatology and immunology department is an important branch of internal medicine,which is a compulsory department in the rotation of internal medicine.Rheumatism is one of the departments with high difficulty in the standardized training of resident doctors because of its diverse clinical manifestations,abstract content and easy intersection with other disciplines.This paper summarizes the teaching experiences of the department over the years,including the following aspects:(1)Improve the entrance education and master the basic knowledge of rheumatology;(2)Enrich clinical teaching and master the diagnosis and treatment of common rheumatism by adopting clinical path method,the combination of three tracks of LBL(lecture-based learning),CBL(case-based learning)and PBL(problem-based learning),and the application of micro-course flipped classroom teaching mode and other modes;(3)Increase practical operation experience and attach importance to the cultivation of clinical practice skills;(4)Improve the overall clinical thinking of physicians by teaching rounds to patients with difficult medical records;(5)“Weekly lectures”to improve students'independent learning ability.Through the above aspects of teaching,using a variety of teaching means,guide the regular students to master the requirements of training content,gradually form a good overall clinical thinking,give play to the ability of independent learning,improve the quality of the routine teaching of rheumatology and immunology department,so as to improve the comprehensive ability of the students and grow into qualified clinicians.
