摘要 目的研究程序性死亡配体1(PD-L1)、Beclin-1、P62在非小细胞肺癌(NSCLC)中的表达与复发转移的关系。方法选取2019年7月至2020年6月三峡大学附属仁和医院检验科收治的88例NSCLC患者作为病例组,另选距离病例组癌组织5 cm以上的癌旁组织作为对照组,采用免疫组化法检测两组PD-L1、Beclin-1、P62的表达情况,采用Cox分析NSCLC患者无复发生存情况的影响因素。结果病例组PD-L1、P62的阳性率均高于对照组,Beclin-1的阳性率低于对照组,差异均有统计学意义(P<0.05)。不同肿瘤直径、临床分期、分化程度和吸烟史的NSCLC患者的PD-L1、Beclin-1阳性率比较,差异有统计学意义(P<0.05);不同淋巴结转移情况、肺膜浸润的NSCLC患者的Beclin-1、P62阳性率比较,差异有统计学意义(P<0.05);不同肿瘤直径的NSCLC患者的P62阳性率比较,差异有统计学意义(P<0.05)。末次随访,88例NSCLC患者复发16例,复发率为18.18%。临床分期、淋巴结转移和P62阳性是影响NSCLC患者无复发生存情况的独立因素(P<0.05)。结论NSCLC患者PD-L1、Beclin-1、P62表达异常,且P62阳性是影响NSCLC患者复发转移的独立因素。 Objective To study the expression of programmed death ligand 1(PD-L1),Beclin-1 and P62 in non-small cell lung cancer(NSCLC)and their relationship with recurrence and metastasis.Methods A total of 88 patients with NSCLC admitted to the Department of Clinical Laboratory in Renhe Hospital Affiliated to China Three Gorges University were enrolled as the case group from July 2019 to June2020,and the paracancerous tissue more than 5 cm away from the cancer tissue in the case group was selected as the control group,and immunohistochemistry was used.The expression of PD-L1,Beclin-1 and P62 in the two groups were detected by immunohistochemistry.The influencing factors of recurrence-free survival(RFS)in NSCLC patients were analyzed by Cox.Results The positive rates of PD-L1 and P62 in the case group were higher than those in the control group,while the positive rate of Beclin-1 was lower than that in the control group(P<0.05).The differences in positive rates of PD-L1 and Beclin-1 among NSCLC patients with different tumor diameters,clinical staging,differentiation degree and smoking history were statistically significant(P<0.05).There were significant differences in the positive rates of Beclin-1 and P62 in NSCLC patients with different lymph node metastasis and pulmonary membrane infiltration(P<0.05).The differences in positive rate of P62 among NSCLC patients with different tumor diameters were statistically significant(P<0.05).At the last follow-up,16 cases of 88 NSCLC patients relapsed,with a recurrence rate of 18.18%.Clinical staging,lymph node metastasis and P62 positive were independent factors affecting RFS in NSCLC patients(P<0.05).Conclusion The expression of PD-L1,Beclin-1 and P62 are abnormal in NSCLC patients,and P62 positive is an independent factor affecting the recurrence and metastasis of NSCLC patients.
