摘要
目的探讨乳酸菌联合低聚果糖、腹式呼吸法治疗老年功能性便秘的疗效及机制。方法选取2019年2—8月雅安市第四人民医院收治的功能性便秘患者84例,用随机数字表分为对照组和研究组,各42例。对照组患者予以低聚果糖+腹式呼吸法治疗,研究组患者予以乳酸菌+低聚果糖+腹式呼吸法治疗。对比治疗前后两组便秘症状评分、临床疗效,以及血浆中胃动素(MTL)、生长抑素(SS)、血清神经肽Y(NPY)的变化、肠道菌群变化和不良反应发生率。结果治疗后,两组便秘症状评分均降低(P<0.01),且研究组更低(P<0.01),研究组总有效率为95.24%,显著高于对照组的78.57%(P<0.05)。治疗后,两组血浆中MTL均升高(P<0.01),且研究组高于对照组(P<0.01);血浆SS、血清NPY含量均降低(P<0.01),且研究组均低于对照组(P<0.01)两组ACE指数、Shannon指数均升高且研究组高于对照组,两组双歧杆菌属、肠球菌属、乳杆菌属、消化链球菌属相对丰度均变大且研究组更大,拟杆菌属、肠杆菌术相对丰度均降低且研究组更低,以上对比差异均有统计学意义(P<0.01)。研究组治疗期间不良反应发生率为11.90%,对照组为9.52%,差异无统计学意义(P>0.05)。结论乳酸菌联合低聚果糖、腹式呼吸法治疗老年功能性便秘不仅能有效改善患者便秘症状、增强临床疗效,还能降低血浆中MTL水平、增大血浆SS、血清NPY含量、改善患者肠道微生态,安全可靠。
Objective To investigate the therapeutic effect observation and mechanism study of lactic acid bacteria combined with fructose-oligosaccharides and abdominal breathing in the treatment of elderly patients with functional constipation.Methods 84 functional constipation patients treated in the Fourth People’s Hospital of Ya’an from February 2019 to August 2019 were selected and divided into control group and study group with random number table.Forty-two patients in control group were treated with fructoseoligosaccharides and abdominal breathing,while study group were treated with lactic acid bacteria combined with fructoseoligosaccharides and abdominal breathing.The scores of constipation symptoms,clinical efficacy,changes of motilin(MTL),somatostatin(SS)in plasma,serum neuropeptide Y(NPY),changes in intestinal flora and the incidence of adverse reactions were compared between the two groups before and after treatment.Results After treatment,constipation symptom scores were decreased in both groups(control group:t=18.651,P<0.05;study group:t=23.636,P<0.05),and study group was lower(t=17.333,P<0.05).There was a statistically significant difference in the distribution of clinical efficacy between study group and control group(Z=6.732,P<0.05),the total effective rate was higher in study group(χ^(2)=5.126,P<0.05).After treatment,MTL in plasma was increased in both groups(control group:t=9.203,P<0.05;study group:t=10.613,P<0.05)and study group was higher than control group(t=5.069,P<0.05),meanwhile the content of SS()in plasma and NPY(control group:t=5.081,P<0.05;t=18.212,P<0.05;study group:t=10.341,P<0.05;t=23.408,P<0.05)in serum decreased,which was lower in study group(SS:t=3.390,P<0.05;NPY:t=4.629,P<0.05).After treatment,ACE index and Shannon index of both groups were increased,and study group was higher than control group(P<0.05).The relative abundances of Bifidobacteria,Enterococcus,Lactobacillus and digestive Streptococcus in the two groups all increased and were larger in study group,while the relative abundances of Bacteroides and Enterobacterium.decreased and were lower in study group,all the above differences were statistically significant(P<0.05).The incidence of adverse reactions during treatment was 11.90%in study group and 9.52%in control group,with no statistically significant difference between the two groups(correctedχ^(2)=0.000,P>0.05).Conclusion Lactic acid bacteria combined with fructose-oligosaccharides and abdominal breathing in the treatment of elderly functional constipation can not only effectively improve constipation symptoms,enhance clinical efficacy,but also reduce plasma MTL level,increase plasma SS and serum NPY content,as well as improve the intestinal microecology of patients,safe and reliable.
