摘要
目的分析第一跖骨干Scarf截骨结合趾骨Akin截骨治疗中重度踇外翻短期的临床效果。方法选取2018年9月—2019年10月我院收治的16例中重度踇外翻患者,应用第一跖骨干Scarf截骨结合趾骨Akin截骨的手术方式进行矫治,术后6个月对患足进行负重正侧位X线片检查,比较术前及术后踇趾外翻角度(HVA)和第一第二跖骨间夹角(IMA),同时采用美国足踝协会(AOFAS)评分标准及生活质量调查表(SF-36)评分标准评估患者术后恢复情况。结果16例患者随访时间6个月,所有患者术后切口愈合良好,未出现切口感染、踇内翻、踇僵硬等相关并发症。术后6个月复查HVA角及IMA均较术前明显减小(t=22.855、33.358,P<0.05),AOFAS评分、SF-36评分较术前明显升高(t=-26.154、-28.203,P<0.05)。结论第一跖骨干Scarf截骨结合趾骨Akin截骨治疗中重度踇外翻具有矫形能力强、稳定性高的特点,短期临床疗效较好,患者满意度高。
Objective To investigate the clinical effect of Scarf osteotomy of the first metatarsal shaft combined with Akin osteotomy of the phalanx in the treatment of moderate-to-severe hallux valgus.Methods A total of 16 patients with mo-derate-to-severe hallux valgus who were admitted to our hospital from September 2018 to October 2019 were enrolled and treated with the surgical method of Scarf osteotomy of the first metatarsal shaft combined with Akin osteotomy of the phalanx.Anteroposterior and lateral X-ray films of the diseased foot under weight bearing were obtained at 6 months after surgery,and hallux valgus angle(HVA)and intermetatarsal angle(IMA)were compared before and after treatment.The American Orthopedic Foot and Ankle Society(AOFAS)scoring standards and the quality of life questionnaire(SF-36)were used to assess the postoperative recovery of patients.Results The 16 patients were followed up for 6 months,and all patients had good incision healing after surgery,without the complications such as incision infection,hallux varus,and hallux stiffness.Reexamination at 6 months after surgery showed significant reductions in HVA and IMA(t=22.855,33.358,P<0.05)and significant increases in AOFAS score and SF-36 score after surgery(t=-26.154,-28.203,P<0.05).Conclusion Scarf osteotomy of the first metatarsal shaft combined with Akin osteotomy of the phalanx has the advantages of strong correction ability,high stability,good short-term clinical effect,and a high degree of patient satisfaction in the treatment of moderate-to-severe hallux valgus.
