摘要
目的探讨腹腔镜结合Bianchi手术治疗低位隐睾的应用价值。方法2018年6月~2020年12月,对22例小儿低位隐睾采用腹腔镜结合Bianchi手术治疗。腹腔镜探查如存在鞘状突未闭,先行高位结扎。结果22例手术均获成功,13例单侧隐睾中7例患侧鞘状突未闭,2例对侧鞘状突未闭;9例双侧隐睾中5例双侧鞘状突未闭,1例单侧鞘状突未闭。单侧隐睾手术时间20~30 min,双侧隐睾手术时间35~40 min,术中出血均<5 ml。随访1~12个月,平均6个月,超声提示双侧睾丸位于阴囊内,无鞘膜积液、精索血肿、睾丸萎缩等并发症。结论腹腔镜结合Bianchi手术治疗低位隐睾,既保留Bianchi手术的优点,又容易达到下降睾丸至阴囊底及高位结扎鞘状突的目的。
Objective To explore the application value of laparoscopy combined with Bianchi operation in the treatment of low cryptorchidism.Methods From June 2018 to December 2020,22 children with low cryptorchidism were treated by laparoscopy combined with Bianchi operation.If the patent sheath process was found in laparoscopic exploration,high ligation was carried out firstly.Results The operation was successful in all the 22 cases.Among 13 cases of unilateral cryptorchidism,7 cases had ipsilateral patent sheath process and 2 cases had contralateral patent sheath process.Among the 9 cases of bilateral cryptorchidism,5 cases had bilateral patent sheath process and 1 case had unilateral patent sheath process.The operation time of unilateral cryptorchidism was 20-30 min and that of bilateral cryptorchidism was 35-40 min.The intraoperative bleeding was less than 5 ml.The patients were followed up for 1-12 months,with an average of 6 months.Ultrasonography showed that the bilateral testicles were located in the scrotum,and there were no complications such as hydrocele,spermatic cord hematoma or testicular atrophy.Conclusion Laparoscopy combined with Bianchi operation in the treatment of low cryptorchidism not only retains the advantages of Bianchi operation,but also is easy to lower the testis to the scrotum bottom and ligate the sheath process at a high position.
