摘要
目的探讨口服降糖药二甲双胍联合常规胰岛素治疗妊娠期糖尿病的效果及对孕妇炎症因子和新生儿指标的影响。方法选取2019年1月—2020年4月盘锦市中心医院收治的64例妊娠糖尿病孕妇为研究对象,随机分为对照组和观察组各32例。对照组行常规胰岛素治疗,观察组行口服二甲双胍联合胰岛素治疗。对比两组治疗前后的血糖水平和炎症细胞因子水平,以及新生儿出生指标等方面的差异。结果治疗后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白均低于对照组,差异均有统计学意义(P<0.05)。观察组高敏C-反应蛋白和白细胞介素-6水平均低于对照组,差异均有统计学意义(P<0.05)。观察组新生儿体质量、体长和头围优于对照组,低血糖和黄疸发生率低于对照组,差异均有统计学意义(P<0.05)。结论口服降糖药二甲双胍联合胰岛素治疗妊娠期糖尿病,较单独使用胰岛素治疗,更有利于维持孕妇血糖水平及炎症细胞因子水平的稳定性,同时,可进一步改善新生儿体质量、体长等出生指标,并降低低血糖和黄疸的发生率,值得推广使用。
Objective To investigate the effect of oral metformin hydrochloride combined with insulin on pregnancy outcome and neonatal indicators in patients with T2DM at the end of pregnancy.Methods A total of 64 patients with gestational T2DM between January 2019 and April 2020 in Panjin Central Hospital were randomly divided into control group and observation group(32 cases in each group).The control group was treated with routine insulin therapy,and the observation group was treated with oral metformin combined with insulin therapy.The differences of blood glucose level,inflammatory cytokines and neonatal indicators between the two groups were compared.Results After treatment,compared with the control group,the levels of fasting plasma glucose,2 hour postprandial blood glucose and glycosylated hemoglobin A-1c in the observation group were significantly lower than those in the control group(P<0.05).Serum high sensitivity C-reactive protein and lnterleukin-6 in the observation group were significantly lower than those in the control group(P<0.05).The neonatal weight,body length,head circumference,hypoglycemia and jaundice were significantly different between the two groups(P<0.05).Conclusion Metformin combined with insulin can maintain stable levels of blood glucose and inflammatory cytokines,and significantly improve neonatal weight,body length and other neonatal indicators,as well as the incidence of hypoglycemia and jaundice in patients with gestational diabetes mellitus.
