摘要
目的探究肿瘤患者在留置PICC期间做出过哪些损伤导管的行为,利用计划行为理论分析这些行为产生的原因,并提出相应的解决办法。方法采用目的抽样法,选取2021年1月至2021年3月在锦州医科大学附属第一医院置入PICC导管2个月以上的患者为研究对象。基于计划行为理论建立访谈提纲,对患者进行半结构式访谈,根据访谈结果分析导致患者产生PICC损伤行为的原因。结果本次访谈中所有访谈对象在留置导管期间均有过损伤导管的行为;应用计划行为理论对患者PICC导管损伤行为进行分析结果如下:(1)行为态度维度:感到麻烦、对疾病的消极态度和对后果的认识不足;(2)主观规范:既往经验、家人的言行和他人因素;(3)知觉行为控制:到院维护不便、家庭和工作中有事、经济状况差及行动不方便。结论在完善其维护导管的知识、提高患者维护导管的意识、增强其对治疗的信心的基础上,根据患者需求改善治疗环境是减少患者留置导管期间损伤行为发生的重要方式。
Objective To explore the behaviors of catheter damage of tumor patients during indwelling PICC,analyze the causes of these behaviors by using the theory of planned behavior,and propose corresponding solutions.Methods Patients with PICC catheter implantation for more than 2 months in the First Affiliated Hospital of Jinzhou Medical University from January 2021 to March 2021 were selected as subjects by purpose sampling method.Based on the theory of planned behavior,an interview outline was established to conduct semi-structured interviews with patients,and the causes of patients'PICC injury behaviors were analyzed according to the interview results.Results All interviewees in this interview had damaged catheter during indwelling catheter.The theory of planned behavior was applied to analyze the PICC catheter damage behavior of patients,and the results were as follows:(1)behavioral attitude dimension:feeling troubled,with negative attitude towards disease and insufficient understanding of consequences;(2)subjective norms:previous experience,words and deeds of family members and other factors;(3)perceptual behavioral control:inconvenient maintenance,family and work problems,poor economic status and inconvenient movement.Conclusion On the basis of improving their knowledge of catheter maintenance,strengthening their awareness of catheter maintenance and enhancing their confidence in treatment,improving the treatment environment according to patients'needs is an important way to reduce damage behaviors during the period of indwelling catheter.
