妊娠期糖尿病孕妇产后发生糖代谢异常危险因素的Meta分析 被引量:1

Meta-analysis of Risk Factors for Abnormal Glucose Metabolism in Pregnant Women with Gestational Diabetes

摘要 目的探讨妊娠期糖尿病孕妇产后发生糖代谢异常的危险因素。方法计算机检索CNKI、WanFang Data、CBM、VIP、PubMed、Web of Science、EMbase和The Cochrane Library数据库,搜集有关妊娠期糖尿病孕妇产后发生糖代谢异常危险因素的研究,检索时间均建库至2021年8月。由两位研究者独立筛选文献、提取资料,并按照纳入排除标准对文献进行质量评价,通过RevMan 5.3软件进行Meta分析。结果共纳入17个研究,共计4 444例研究对象,涉及7个危险因素。Meta分析结果表明:产后发生糖代谢异常组在孕前体质指数[OR=2.03,95%CI(1.56~2.63)]、糖尿病家族史[OR=3.06,95%CI(2.06~4.54)]、年龄[OR=1.14,95%CI(1.05~1.23)]、孕中期三酰甘油[OR=1.34,95%CI(1.21~1.48)]及孕期葡萄糖耐量试验后2 h血糖[OR=1.39,95%CI(1.18~1.65)]暴露因素上,差异有统计学意义(P<0.05);而在诊断孕周[OR=2.23,95%CI(0.73~6.84)]、2 h胰岛素水平[OR=0.98,95%CI(0.95~1.01)]暴露因素上,差异无统计学意义(P>0.05)。结论孕前体质指数、糖尿病家族史、年龄、孕中期三酰甘油及孕期葡萄糖耐量试验后2 h血糖是妊娠期糖尿病孕妇产后发生糖代谢异常的危险因素。 Objective To explore the risk factors of abnormal glucose metabolism in pregnant women with gestational diabetes. Methods Computer search of CNKI, WanFang Data, CBM, VIP, PubMed, Web of Science, EMbase and Cochrane Library databases. Collect the research on the risk factors of abnormal glucose metabolism in pregnant women with gestational diabetes, and the search time will be established until August 2021. Two researchers independently screened the literature, extracted the data, and evaluated the quality of the literature according to the inclusion and exclusion criteria.Meta-analysis was performed by RevMan 5.3 software. Results A total of 17 studies were included, with a total of 4 444 subjects, involving 7 risk factors. Meta-analysis results showed that: the body mass index [OR=2.03, 95%CI(1.56~2.63)],family history of diabetes [OR=3.06, 95%CI(2.06~4.54)], age [OR=1.14, 95%CI(1.05~1.23)], second trimester triglycerides[OR=1.34, 95%CI(1.21~1.48)] and blood glucose 2 hours after pregnancy glucose tolerance test [OR=1.39, 95%CI(1.18~1.65)], in terms of exposure factors, the difference was statistically significant(P<0.05). However, there was no statistically significant difference in the exposure factors at the diagnosis gestational week [OR =2.23, 95% CI(0.73~6.84)] and 2-hour insulin level [OR=0.98, 95%CI(0.95~1.01)](P>0.05). Conclusion Pre-pregnancy body mass index, family history of diabetes, age, second trimester triglyceride and 2 hours blood glucose after pregnancy glucose tolerance test are risk factors for abnormal glucose metabolism in pregnant women with gestational diabetes.
