摘要
目的探讨喹硫平联合认知行为疗法治疗阿尔茨海默病的临床疗效及安全性。方法将100例阿尔茨海默病患者按随机数字表法分为两组,各50例。对照组给予喹硫平治疗,研究组在对照组基础上联合认知行为疗法治疗,观察3个月。比较两组的临床疗效、不良反应发生率,治疗前后阿尔茨海默病行为病理评定量表、痴呆抑郁量表、简明精神病量表评分及脑损伤标志物(神经元烯醇化酶、胶质纤维酸性蛋白、同型半胱氨酸)水平。结果研究组总有效率显著高于对照组(P<0.05)。治疗后研究组阿尔茨海默病行为病理评定量表、痴呆抑郁量表、简明精神病量表评分显著低于对照组(P<0.01),血清同型半胱氨酸、神经元烯醇化酶、胶质纤维酸性蛋白水平显著低于对照组(P<0.01)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论认喹硫平联合认知行为疗法治疗阿茨海默病疗效显著,能够改善患者的精神行为症状,降低脑损伤水平,安全性高。
Objective To investigate the clinical efficacy and safety of quetiapine combined with cognitive behavioral therapy in the treatment of Alzheimer's disease.Methods A total of 100 patients with Alzheimer's disease were divided into two groups according to random number table method,with 50 cases in each group.The control group was treated with quetiapine,and the study group was treated with cognitive behavioral therapy on this basis foR3 months.The clinical efficacy,the incidence of adverse reactions,the scores of behavioral pathology assessment scale of Alzheimer's disease(BEHAVE-AD),dementia depression scale(CSDD),brief psychiatric rating scale(BPRS)and the levels of brain injury markers neuron enolase(NSE),glial fibrillary acidic protein(GFAP),homocysteine(Hcy)were compared between the two groups before and after treatment.Results The total effective rate of the study group was significantly higher than that of the control group(P<0.05).After treatment,the scores of BEHAVE-AD,CSDD and BPRS in the study group were significantly lower than those in the control group(P<0.01),and the levels of serum Hcy,NSE and GFAP were significantly lower than those in the control group(P<0.01).Conclusions Quetiapine combined with cognitive behavioral therapy is effective in the treatment of Alzheimer's disease.It can improve the mental and behavioral symptoms of patients,reduce the level of brain injury and has high safety.
