摘要
术后疼痛控制不仅有助于缓解患者术后的恐惧感,提升手术治疗的效果,还可以尽量减少围手术期并发症的发病率,甚至降低病人的住院成本,因此文章探讨了利多卡因联合舒芬太尼在乳房切除术后的术后镇痛效果。选择2019年1月至2019年12月于该院普外科全麻醉下择期行乳房切除手术患者82例,按照随机分组原则,将其随机分成实验组和对照组,每组41名。实验组经自动镇痛泵注入0.25%利多卡因联合0.25μg/mL舒芬太尼注射液混合液10 mL,连续5 min。对照组经自动镇痛泵只注入0.25%利多卡因注射液10 mL,持续5 min。记录术后患者的在手术即刻、术后1,24和48 h的HR、SBp、DBp、MAp等生理指标和采用VAS评分评估患者的疼痛程度以及记录术后不良反应情况。结果显示,两组患者术后镇痛过程中的HR,SBp、DBp、MAp在术后即刻,24 h和48 h均无明显差异;在手术后的即刻VAS评分,两组间无明显差异,而在术后的1,12,24和48 h的VAS评分与术后即刻评分相比,均呈显著性降低(P<0.05),而且与对照组相比,实验组患者的VAS评分显著降低,两组差异具有统计学意义(P<0.05);实验组在镇痛过程中出现不良反应5例(12%),显著低于对照组的16例(32%),差异具有统计学意义(P<0.05)。对于乳房切除术后疼痛的患者给予利多卡因联合舒芬太尼镇痛可以达到良好的镇痛效果,因为其具有副作用小,无用药依赖性,并且镇痛持续时间长。
Postoperative pain control not only helps to alleviate patients’postoperative fears and enhance the effectiveness of surgical treatment,but also minimizes the incidence of perioperative complications and even reduces patients’hospitalization costs,therefore,this study explores the postoperative analgesic effect of lidocaine combined with sufentanil after mastectomy.82 patients who underwent elective mastectomy under general surgery with general anesthesia at this hospital from January 2019 to December 2019 were selected,and based on the randomized control principle,the 82 patients were randomly divided into study group and control group,with 41 patients in each group.The study group was injected with 0.25%lidocaine combined with 0.25μg/mL sufentanil injection mixture 10 mL via automatic analgesic pump for 5 min,and the control group was injected with 0.25%lidocaine injection only 10 mL via automatic analgesic pump for 5 min.Physiological indices such as DBp,MAp and VAS scores were used to assess patients’pain levels and to record postoperative adverse effects.The results showed that the HR,SBp,DBp,and MAp during postoperative analgesia were not significantly different between the two groups at the immediate postoperative period,24 h,and 48 h.The VAS scores at 1 h,12 h,24 h,and 48 h were significantly different between the two groups compared with the immediate postoperative scores.Compared with the control group,the VAS score of patients in the study group was significantly lower(P<0.05),and the difference between the two groups was statistically significant(P<0.05).5 cases(12%)of adverse reactions in the analgesic process in the study group were significantly lower than 16 cases(32%)in the control group,and the difference was statistically significant(P<0.05).Lidocaine combined with sufentanil for analgesia in patients with post-mastectomy pain could achieve good analgesic effect because it had few side effects,no drug dependence,and long duration of analgesia,which could be promoted clinically.
