摘要
目的探究不同椎管内麻醉方法对老年下肢骨折患者循环功能的影响。方法选取2018年7月至2019年7月于本院行手术治疗的1500例老年下肢骨折患者作为研究对象,根据麻醉方式不同分为研究组与对照组,每组750例。研究组实施腰-硬联合麻醉,对照组实施持续硬膜外麻醉。比较两组麻醉后血压、心率及术中输液情况。结果麻醉10 min时(T1),研究组平均动脉压(MAP)、收缩压(SBP)、舒张压(DBP)、心率(HR)水平波动幅度小于对照组(P<0.05)。两组晶体液输入量、胶体液输入量、初始500 mL液体输注时间、平均每小时输液量及总输液量比较差异无统计学意义。结论老年下肢骨折患者手术治疗中腰-硬联合麻醉及持续硬膜外麻醉均会对患者的循环功能产生一定影响,但腰-硬联合麻醉的应用效果明显较好,稳定性更佳,循环波动较小。
Objective To explore the effects of different intraspinal anesthesia methods on the circulatory function in elderly patients with lower extremity fractures.Methods 1500 elderly patients with lower extremity fractures who underwent surgical treatment in our hospital from July 2018 to July 2019 were selected as the research subjects.According to the different anesthesia methods,they were divided into study group and control group,with 750 cases in each group.The study group received combined spinal-epidural anesthesia,and the control group received continuous epidural anesthesia.The blood pressure,heart rate and intraoperative fluid infusion between the two groups after anesthesia were compared.Results At 10 minutes of anesthesia(T1),the mean arterial pressure(MAP),systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate(HR)of the study group fluctuated less than those of the control group(P<0.05).There was no statistically significant difference between the two groups in crystalloid fluid input,colloidal fluid input,initial 500 mL fluid infusion time,average hourly infusion volume and total infusion volume.Conclusion In elderly patients with lower extremity fractures,combined spinal-epidural anesthesia and continuous epidural anesthesia will have certain impact on the patients'circulatory function,but the effect of the combined spinal-epidural anesthesia is significantly better,with better stability and less circulatory fluctuation.
