超声造影检查在C-erbB-2、Ki-67表达阳性及阴性乳腺癌患者中的应用 被引量:2

摘要 目的探讨超声造影检查技术在C-erbB-2、Ki-67表达阳性及阴性乳腺癌患者中的临床意义与价值。方法选择2018年1月—2020年6月于本院就诊的178例乳腺癌患者作为研究对象。所有患者术前均行乳腺超声造影检查及定性定量分析,后采取免疫组化染色技术检测病灶组织C-erbB-2以及Ki-67表达情况,对比C-erbB-2以及Ki-67不同表达情况乳腺癌患者超声造影定性定量参数的差异。结果C-erbB-2、Ki-17表达阳性患者平均年龄、肿瘤直径、病理分期与C-erbB-2表达阴性患者比较无明显差异(P>0.05);C-erbB-2表达阳性患者超声造影检查中增强时间主要表现为快进灌注,与C-erbB-2表达阴性患者比较存在明显差异(P<0.05),而在增强均匀性、增强范围、增强边缘以及充盈缺损表现方面比较无显著差异(P>0.05);Ki-17表达阳性患者超声造影检查中存在充盈缺损构成比与Ki-17表达阴性患者比较存在明显差异(P<0.05),而增强时间、增强均匀性、增强范围、增强边缘定性参数比较无显著差异(P>0.05);C-erbB-2、Ki-17表达阳性、阴性乳腺癌患者超声造影定性参数中峰值强度、达峰时间、曲线下面积与C-erbB-2、Ki-17表达阴性患者比较存在明显差异(P<0.05)。结论C-erbB-2、Ki-67表达效果与超声造影检查定性定量参数之间具有密切联系,通过乳腺超声造影检查可实现肿瘤细胞增殖状态的无创性评估,对乳腺癌诊断及治疗具有重要的指导作用,可为乳腺癌的治疗与预后评估提供参考依据。 Objective To explore the clinical significance and value of contrast-enhanced ultrasound in patients with C-erbB-2,Ki-67 positive and negative breast cancer.Methods 178 patients with breast cancer who were treated in our hospital from January 2018 to June 2020 were selected as the research objects.All patients underwent breast contrast-enhanced ultrasonographyexamination and qualitative and quantitative analysis before the operation,and then adopted immunohistochemical staining technique to detect the expression of C-erbB-2 and Ki-67 in the lesion tissue.Differences in qualitative and quantitative parameters of contrast-enhanced ultrasound in breast cancer patients with different expressions of C-erbB-2 and Ki-67 were compared.Results The average age,tumor diameter,pathological stage of patients with C-erbB-2 and Ki-17 positiveexpression were not significantly different from those with negative C-erbB-2 expression(P>0.05).The enhancement time in the angiography examination in patients with positive C-erbB-2 expressionis mainly manifested as fast-forward perfusion,which is significantly different from that of patients with negative C-erbB-2 expression(P<0.05).However,there were no significant difference in aspect of enhancement uniformity,enhancement range,enhancement edge and filling defect performance between the two groupsof patients(P>0.05).The proportion of filling defects in the contrast-enhanced ultrasound examination of Ki-17 positive patients was significantly different from that of Ki-17 negative patients(P<0.05).There were no significant differences in enhancement time,enhancement uniformity,enhancement range,enhancement edge qualitative parameters(P>0.05).There were statistically significant difference in peak intensity,time to peak,and area under the curveof the qualitative parameterscontrast-enhanced ultrasound examinationbetween the cancer patients with C-erbB-2,Ki-17 positive and negative expression and cancer patients with negative expression of C-erbB-2 and Ki-17(P<0.05).Conclusions The expression effect of C-erbB-2 and Ki-67 is closely related to the qualitative and quantitative parameters of contrast-enhanced ultrasound.Contrast-enhanced breast ultrasound could achieve noninvasive assessment of tumor cell proliferation,which is important diagnosis and treatmentfor breast cancer.It could provide a new basis for the treatment and prognosis assessment of breast cancer.
