摘要
目的探析超声和CTA在颈动脉粥样硬化中的应用意义。方法选取该院2019年11月—2020年11月收治的50例颈动脉粥样硬化患者作为研究对象,依照检查方法依次对患者开展超声检查、CT血管造影检查(CTA)。完成检查后对比两种检查方法的颈总动脉(CCA)、颈外动脉(ECA)、颈内动脉(ICA)斑块检出数量以及斑块检出类型、CCA易损斑块数量、颈动脉中重度狭窄率。结果斑块检出数量方面,CTA检查在ICA斑块检出率上高于超声检查,差异有统计学意义(P<0.05);在CCA斑块检出率、ECA斑块检出率方面,两种检查方法差异无统计学意义(P>0.05)。超声的ICA各类型斑块的检出率均低于CTA检查,差异有统计学意义(P<0.05);在CCA、ECA各类型斑块的检出方面,两种检查方法差异无统计学意义(P>0.05)。对于表面不规则型斑块、溃疡型斑块,CTA检查结果均低于超声检查结果,差异有统计学意义(P<0.05),而在软斑块的检出率上,两者差异无统计学意义(P>0.05)。CTA检查的颈动脉中重度狭窄率检出率高于超声组,差异有统计学意义(P<0.05)。结论在颈动脉粥样硬化进行CTA检查,相比超声检查在ICA斑块检出、颈动脉狭窄诊断方面更具优势,但超声在CCA易损斑块评估中则更具优势,两种检查方法各有自身优缺,临床可互相配合以提高诊断精确度。
Objective To explore the application significance of ultrasound and CTA in carotid atherosclerosis.Methods Fifty patients with carotid atherosclerosis admitted to the hospital from November 2019 to November 2020 were selected as the research objects,and ultrasound examination and CT angiography(CTA)were carried out on the patients in accordance with the examination methods.After completing the inspection,compared the number of plaques detected in the common carotid artery(CCA),external carotid artery(ECA),and internal carotid artery(ICA)of the two inspection methods,as well as the types of plaques detected,the number of vulnerable plaques in CCA,rate of moderate to severe carotid stenosis.Results In terms of the number of plaques detected,the detection rate of ICA was higher by CTA than by ultrasound,and the difference was statistically significant(P<0.05).There was no statistically significant difference between the two methods in CCA plaque detection rate and ECA plaque detection rate(P>0.05).The detection rate of ICA was lower than that of CTA,and the difference was statistically significant(P<0.05).There was no statistically significant difference between the two methods in the detection of CCA and ECA plaques(P>0.05).For surface irregular plaques and ulcer-type plaques,the results of CTA examination were lower than those of ultrasound examination,the difference was statistically significant(P<0.05),while the comparison of the detection rate of soft plaque,the difference was not statistically significant(P>0.05).The detection rate of moderate and severe carotid stenosis in CTA was higher than that in ultrasound group,and the difference was statistically significant(P<0.05).Conclusion CTA examination for carotid atherosclerosis has more advantages in ICA plaque detection and carotid artery stenosis diagnosis than ultrasound examination,but ultrasound has more advantages in the evaluation of CCA vulnerable plaque.Both methods have their own advantages and disadvantages,and can be cooperated with each other clinically to improve the diagnostic accuracy.
