摘要
目的分析右美托咪定预防七氟醚吸入麻醉苏醒期躁动的效果。方法选择该院2019年1月—2020年6月择期手术治疗的68例患儿作为该次研究的对象,患儿手术中均使用七氟醚吸入麻醉,信封法随机分组。对照组34例,麻醉后生理盐水匀速输注;研究组34例,麻醉后静脉输注右美托咪定。比较两组患儿不同时间点心率、平均动脉压水平、苏醒时间、疼痛评分及苏醒期躁动情况。结果研究组T2、T3、T4时间点心率(72.83±6.85)、(90.23±8.02)、(88.35±6.79)次/min,平均动脉压水平(128.84±5.38)、(136.09±5.07)、(124.59±5.24)mmHg均优于对照组,差异有统计学意义(t=17.742、7.874、5.038、30.795、11.035、22.688,P<0.05);研究组患儿疼痛评分(2.62±0.36)分低于对照组,差异有统计学意义(t=10.168,P<0.05);研究组的苏醒期躁动发生率(7.32%)低于对照组(24.39%),差异有统计学意义(χ^(2)=4.660,P<0.05);研究组苏醒期不良反应发生率为5.89%,低于对照组,差异有统计学意义(χ^(2)=5.775,P<0.05)。结论右美托咪定可以有效预防七氟醚吸入麻醉苏醒期躁动,能够减少苏醒期躁动的发生,且患儿血流动力学稳定,不会延长麻醉苏醒时间,而且安全性高,不良反应少。
Objective Analyze the effect of dexmedetomidine on preventing restlessness during recovery from sevoflurane inhalation anesthesia.Methods 68 children with elective surgical treatment in the hospital from January 2019 to June 2020 were selected as the objects of this study.All children were treated with sevoflurane inhalation anesthesia during the operation,and they were randomized by the envelope method.In the control group,34 patients received a uniform infusion of normal saline after anesthesia;in the study group,34 patients received an intravenous infusion of dexmedetomidine after anesthesia.The snack rate,average arterial pressure level,recovery time,pain score and restlessness during recovery period were compared between the two groups of children at different times.Results The heart rate(72.83±6.85)times/min,(90.23±8.02)times/min,(88.35±6.79)times/min and average arterial pressure(128.84±5.38)mmHg,(136.09±5.07)mmHg,(124.59±5.24)mmHg at T2,T3 and T4 points in the study group were better than those in the control group,the difference was statistically significant(t=17.742,7.874,5.038,30.795,11.035,22.688,P<0.05).The pain score of the study group(2.62±0.36)points was lower than that of the control group,and the difference was statistically significant(t=10.168,P<0.05).The incidence of agitation in the study group(7.32%)was lower than that in the control group(24.39%),and the difference was statistically significant(χ^(2)=4.660,P<0.05).The incidence of adr in the study group was 5.89%,which was lower than the control group,and the difference was statistically significant(χ^(2)=5.775,P<0.05).Conclusion Dexmedetomidine can effectively prevent agitation during the recovery period of sevoflurane inhalation anesthesia,and can reduce the occurrence of agitation during the recovery period.In addition,the patient's hemodynamics is stable,it will not prolong the awakening time from anesthesia,and it has high safety and few adverse reactions.
