摘要
目的达格列净联合沙库巴曲缬沙坦对糖尿病肾病(DN)合并高血压(HTN)患者血压、血糖、肾功能情况的影响。方法回顾性分析2020年1月—2021年1月该院收治的DN合并HTN的80例患者的基本资料,按治疗方法随机抽样选取的病历,分为治疗组和对照组。对照组给予达格列净口服,治疗组给予达格列净联合沙库巴曲缬沙坦口服,每组40例。连续治疗3个月,治疗前后对两组血压、肾功能相关指标[尿素氮(BUN)、肌酐(Cr)、24 h尿蛋白]及血糖检测指标[血糖空腹值(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)]进行检测及比较。结果治疗3个月后,两组不良反应发生率比较,差异无统计学意义(P>0.05)。两组血压、血糖、BUN、Cr、24 h尿蛋白较治疗前显著降低,差异有统计学意义(P<0.05),且治疗组血压、BUN、Cr、24 h尿蛋白低于对照组,差异有统计学意义(P<0.05)。结论沙库巴曲缬沙坦联合达格列净控释片治疗2型DN合并HTN患者,能在一定程度上起到的降血压、降血糖、减少尿蛋白、改善肾功能的作用。
Objective The effect of dapagliflozin combined with sacubitril and valsartan on blood pressure,blood sugar and renal function in patients with diabetic nephropathy(DN) and hypertension(HTN).Methods A retrospective analysis of 80 patients with DN and hypertension admitted to the hospital from January 2020 to January 2021 were the research objects.According to the medical records randomly selected by the treatment method,they were divided into a treatment group and a control group.The control group was given dapagliflozin orally,and the treatment group was given dapagliflozin combined with sacubitril and valsartan,40 cases in each group.The treatment was continued for 3 months,and the blood pressure and renal function related indexes [urea nitrogen(BUN),creatinine(Cr),24 h urine protein] and blood glucose test indicators[glucose fasting value(FPG),2 h postprandial blood glucose(2 hPG),glycosylated hemoglobin(HbA1 c)] of the two groups were tested and compared before and after treatment.Results After 3 months of treatment,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The blood pressure,blood sugar,urea nitrogen(BUN),creatinine(Cr),and 24 h urine protein in the two groups were significantly lower than before treatment,the difference was statistically significant(P<0.05) and blood pressure,urea nitrogen(BUN),creatinine(Cr),and 24 h urine protein in the treatment group were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion In the treatment of patients with type 2 diabetic nephropathy(DN) and hypertension(HTN),sakubatrevalsartan combined with daglijing controlled-release tablets can reduce blood pressure,blood glucose,urinary protein and improve renal function to a certain extent.
