摘要
目的分析2型糖尿病肾病患者各个分型的病例特征和尿蛋白的关系。方法选取莆田学院附属医院和联勤保障部队第九00医院2019年3月-2020年3月收治的60例患者,按照24 h尿蛋白水平分为A(<2000 mg)、B(2000~4000mg)、C(4000~6000 mg)、D(>6000 mg)4组。检测尿素氮(BUN)、血肌酐(Scr)、白蛋白(Alb),观察肾组织染色结果、超微结构及肾小球基底膜厚度水平。结果A组和其他3组进行对比,Alb水平升高较为明显,差异有统计学意义(P<0.05);D组Scr升高和其他3组相比更为明显,BUN水平升高较为明显,和A组对比差异明显,Alb水平和B组、C组比降低程度更为明显,差异有统计学意义(P<0.05)。糖尿病肾病患者的病理分级严重程度和蛋白尿水平为正比关系。D组GBM厚度厚于A组,C组GBM厚度厚于A组,D组GBM厚度厚于B组,提示患者肾小球基底膜厚度会和蛋白尿水平为正比关系。结论糖尿病肾病患者肾脏功能的病变会受到蛋白尿的影响,通过对患者实施有效的病理分型,能够更好地了解和认识患者病情状况,可以为疾病诊断和后期治疗计划制订提供数据基础。
Objective To analyze the relationship between urinary protein and case characteristics in patients with type 2 diabetic nephropathy.Methods 60 patients admitted to the Affiliated hospital of Putian University and the Joint Logistics Support Force No.900 hispital from March 2019 to March 2020 were selected and divided into four groups according to 24-hour urinary protein level:A(<2000 mg),B(2000-4000 mg),C(4000-6000 mg)and D(>6000 mg).Urea nitrogen(BUN),serum creatinine(Scr)and albumin(Alb)were detected.The staining results,ultrastructure and thickness of glomerular basement membrane were observed.Results Compared with the other three groups,Alb level in group A increastd significantly,the difference was statistically significant(P<0.05);compared with the other three groups,the increase of Scr and BUN level in group D was more obvious,and the difference was obvious compared with group A,and the decrease of Alb level was more obvious than that in group B and C,the difference was statistically significant(P<0.05).The severity of pathological grade in diabetic nephropathy patients was proportional to proteinuria level.The thickness of group D GBM was thicker than that of group A,group C GBM thicker than that of group A,and group D GBM thicker than that of group B,reminder:the glomerular basement membrane thickness is proportional to proteinuria level.Conclusion The pathological changes of renal function in patients with diabetic nephropathy will be affected by proteinuria.Through effective pathological typing of patients,we can better understand and understand the condition of patients,and provide data basis for disease diagnosis and later treatment planning.
