摘要
目的分析武汉市武昌区水果湖街新型冠状病毒肺炎(COVID-19)流行特点及预后,为COVID-19患者制定康复措施及调整社区整体疫情防控策略提供理论依据。方法从湖北省基本公共卫生服务管理系统收集水果湖街2020-01-02-2020-05-06出院的确诊COVID-19病例资料,对康复患者进行随访,并分析流行病学特征。结果武昌区水果湖街报告确诊COVID-19病例616例。发病至确诊时间0~77d,平均11.0d;发病至出院时间2~122d,平均住院天数32.3d。发病高峰为1月17日至2月7日;发病年龄4~89岁,平均年龄(56.99±15.65)岁;男310例,女306例,男女之比为1.01∶1;离退休人员280例(45.5%),医务人员46例(7.5%)。其中轻型293例(47.6%),普通型207例(33.6%),重型104例(16.9%),危重型12例(1.9%)。死亡58例(9.4%),年龄43~89岁,平均年龄(73.0±10.6)岁;男33例(56.9%),女25例(43.1%);发病至死亡平均生存天数为21.1d。其中轻症16例(27.6%),普通型13例(22.4%),重型23例(39.7%),危重型6例(10.3%)。年龄≥50岁、离/退休人员及重症/危重症是影响COVID-19患者预后的因素。截至5月30日报告无症状感染者19例,4月9日到23日随诊核酸复阳4例,均无症状。结论武汉市武昌区水果湖街辖区COVID-19发病率为0.71%,死亡病例中离退休人员和年长者居多,确诊COVID-19患者年龄≥50岁、离/退休人员及重症/危重症和合并慢性病者预后差。
Objective To analyze the epidemic characteristics and prognosis of the coronavirus disease 2019(COVID-19)in Shuiguohu Street,Wuchang District,Wuhan City,and provide a theoretical basis for COVID-19 patients to formulate rehabilitation measures and adjust the overall community epidemic prevention and control strategies.Methods To collect the data of confirmed COVID-19 cases discharged from the hospital between January 1 and May 6,2020 from the basic public health service management system of Hubei Province,followed up the recovered patients,and analyzed the epidemiological characteristics.Results A total of 616 cases confirmed with COVID-19 were reported in Shuiguohu community.The time period from onset to diagnosis was 0-77 days,with an average of 11.0 days.The time period between onset and discharging from hospital was 2-122 days,with an average of 32.3 days.The COVID-19 epidemic peak was from January 17 to February 7,the age of COVID-19 confirmed cases was 4-89 years old,with the average age(56.99±15.65)years old.There were 310 males and 306 females,the ratio of male to female was 1.01∶1.There were 280 retired persons(45.5%)and 46 medical staff(7.5%).Among them,there were 293 cases(47.6%)of light type,207 cases(33.6%)of common type,104 cases of severe type(16.9%),and 12 cases of critical type(1.9%).There were 58 deaths(9.4%),aged 43-89 years old,with an average age of(73.0±10.6)years old,33 males(56.9%)and 25 females(43.1%).The average survival time from onset to death was 21.1 days.Among them,16 cases were mild(27.6%),13 cases were normal(22.4%),23 cases were severe(39.7%),and 6 cases were critical(10.3%).Age≥50 years old,retired/retirees,and severe/critical illness were factors that affected the prognosis of COVID-19 patients.As of May 30,19 cases of asymptomatic infection were reported,and 4 cases of nucleic acid recovery were followed up from April 9 to 23,all of which were asymptomatic.Conclusions The incidence of COVID-19 in Shuiguohu community is 0.71%.In community,the majority of death cases are elderly retirees.Age over 50 years old,retirees,severe/critical of clinical classification and comorbidities are the main factors affecting the death prognosis in COVID-19 patients.
