摘要
目的分析儿童右肺中叶综合征的病因及病原类型方法对经肺部CT诊断为右肺中叶综合征的38例患儿进行电子支气管镜检查,镜下行支气管肺泡灌洗,观察电子支气管镜下表现,对炎症性因素所致的右肺中叶综合征将肺泡灌洗液进行宏基因二代测序检测,分析其检测结果结果电子支气管镜下检查:炎症引起33例,异物5例33例炎症引起的RMLS患儿肺泡灌洗液的病原微生物宏基因二代测序检测结果中,细菌阳性22例,占66.7%,病原分别为肺炎链球菌(9例)、流血嗜血杆菌(8例)、卡他莫拉菌(3例)、肺炎克雷伯菌(1例)、大肠埃希菌(1例);肺炎支原体5例,占15.2%;腺病毒6例,占18.5%;真菌5例,占15.2%。结论炎症是导致儿童RMLS的主要因素,细菌是主要的病原体,其中以肺炎链球菌及流感嗜血杆菌多见。
Objective To analyze the main etiology and pathogen types of children with the right middle lobe syndrome.Methods 38 children diagnosed as the right nuddle lung syndrome by lung CT were examined with electronic bronchoscopy and bronchoalveolar lavage.The manifestations of electronic bronchoscopy were counted,and the alveolar lavage fluid of the right middle lung syndrome which caused by inflammatory factors were tested for met«igene next-generation sequencing.Finally,the results wereanalyzed.Results Electronic bronchoscopy showed that 33 cases caused by inflammation,and 5 cases caused by bronchialtoreign bodies.The results of metagene next-generation sequencing of alveolar lavage fluid in 33 children with the right middle lung syndrome which caused by inflammation showed that 22 of 33 c.ises(66.7%)were bacteria positive,of which 9 cases were Streptococcus pneumoniae,9 cases were Haemophilus influenz«ie,3 cases wereMoraxelle catarrhalisl case was Klebsiella pneumoniae;l case was Escherichia coli;5 of 33 c.ises(15.2%)were mycoplasma pneumoniae;6cases were Adenovirus,accounting for 18.5%;5 cases were fungi,accounting for 15.2%.Conclusion Inflammation is the main factor of the middle of the right lung syndrome in children.Bacteria is the main cause,of which Streptococcus pneumoniae and Haemophilus influenzae are the most important etiological cause.
