摘要
目的探讨超声二维剪切波弹性成像(SWE)联合实时组织弹性成像(RTE)对小儿先天性肌性斜颈(CMT)的评估价值。方法收集2018年8月至2021年1月河南省儿童医院接收的36例经临床确诊为CMT的患儿临床资料,纳入CMT组;另收集同期医院接收的36例经临床确诊未发生CMT的患儿临床资料,纳入非CMT组。两组患儿均行超声SWE以及RTE检查,比较两组超声SWE参数[杨氏模量最大值(E_(max))、杨氏模量平均值(E_(mean))、弥散度]和RTE参数[弹性图像评分、弹性应变率(SR)],分析超声SWE联合RTE对小儿CMT的评估价值。结果CMT组E_(max)、弥散度值高于非CMT组,差异有统计学意义(P<0.05);CMT组弹性图像评分、SR值均高于非CMT组,差异有统计学意义(P<0.05);E_(max)、弥散度、弹性图像评分以及SR单独及联合检测评估小儿CMT的AUC分别为0.830、0.765、0.763、0.849、0.931,均有一定评估价值,且当E_(max)、弥散度、弹性图像评分、SR分别取99.500 kPa、22.450 kPa、1.615分、1.495时,其敏感度以及特异度较高。结论超声SWE联合RTE对小儿CMT的具有一定评估价值。
Objective To evaluate the value of two-dimensional shear wave elastography(SWE)combined with real-time tissue elastography(RTE)in the evaluation of congenital muscular torticollis(CMT)in children.Methods The clinical data of 36 children who were clinically diagnosed as CMT in Henan children’s Hospital from August 2018 to January 2021 were collected and included in CMT group.In addition,the clinical data of 36 children who were clinically confirmed not to have CMT in the hospital during the same period were collected and included in non CMT group.Children in both groups were examined by ultrasonic SWE and RTE.The ultrasonic SWE parameters[Young’s modulus maximum(E_(max)),Young’s modulus mean(E_(mean)),dispersion]and RTE parameters[elastic image score,elastic strain rate(SR)]were compared between the two groups.The value of ultrasonic SWE combined with RTE in the evaluation of CMT in children was analyzed.Results The values of E_(max) and diffusion in CMT group were higher than those in non CMT group(P<0.05).The elastic image score and SR value in CMT group were higher than those in non CMT group(P<0.05).The AUC of single and combined detection of E_(max),diffusion,elastic image score and SR in evaluating CMT in children were 0.830,0.765,0.763,0.849 and 0.931 respectively,which had certain appraisal value.The sensitivity and specificity were higher when E_(max),dispersion,elastic image score and SR were 99.500 kPa,22.450 kPa,1.615 and 1.495 respectively.Conclusion Ultrasonic SWE combined with RTE has certain value in the evaluation of CMT in children.
