摘要
目的探讨精细化护理模式在凶险性前置胎盘(pernicious placenta previa,PPP)剖宫产术中的应用效果。方法方便选取2018年5月—2020年5月于该院行剖宫产的72例PPP患者为研究对象,随机将研究对象均分为观察组与对照组,各36例。观察组采用常规护理联合精细化护理模式,对照组采用常规护理,观察两组患者术中、术后出血量及手术时间,统计并发症发生率、心率、收缩压、焦虑自评量表(SAS)评分、心理与生理舒适度评分,通过调查问卷的方式了解患者和医生对护理的满意度。结果观察组患者手术时间及术中和术后出血量均低于对照组,差异有统计学意义(P<0.05);观察组患者发生切口感染、低体温及产褥感染等并发症率为11.11%,显著低于对照的30.55%,差异有统计学意义(P<0.05);两组患者在入院时心率及收缩压对比,差异无统计学意义(P>0.05);术中两组患者心率及收缩压均有所升高,且对照组升高较明显,差异有统计学意义(P<0.05);两组患者在入院时SAS评分对比,差异无统计学意义(P>0.05);术后观察组SAS评分显著低于对照组,生理与心理舒适度评分显著高于对照组,差异有统计学意义(P<0.05);观察组患者护理满意度为91.67%,显著高于对照组的72.22%,差异有统计学意义(P<0.05),观察组有9名医生对该次护理非常满意,3名满意,1名一般,对照组有4名医生对护理感觉非常满意,4名满意,2名一般,3名不满意,差异有统计学意义(U=2.064,P=0.039)。结论精细化护理模式在PPP剖宫产手术中能有效缩短手术时间,减少术中与术后出血量,稳定患者心率及收缩压,改善患者焦虑情绪,同时提高患者的心理与生理上的舒适度,降低术后并发症发生率,提升医生与患者的满意度。
Objective To explore the application effect of refined nursing model in pernicious placenta previa(PPP)cesarean section.Methods Selected conveniently 72 PPP patients who were delivered by cesarean section in the hospital from May 2018 to May 2020 as the research objects,and randomly divide the research objects into observation group and control group,36 cases in each group.The observation group adopted conventional nursing combined with refined nursing model,and the control group adopted conventional nursing.The bleeding volume and operation time of the two groups of patients were observed during and after the operation,the incidence of complications,heart rate,sy stolic pressure,self rating anxiety scale(SAS)score,psychdogical and phy siological,comfort score were counted,and the satisfaction of patients and doctors with nursing care was understood through questionnaires.Results The operation time and postoperative blood loss in observation group were lower than those in control group,and the difference was statistically significant(P<0.05).The complication rate of incision infection,hypothermia and puerperal infection in observation group was 11.11%,which was significantly lower than 30.55%in control group,and the difference was statistically significant(P<0.05).There were no statistically significant differences in heart rate and systolic blood pressure between the two groups at admission(P>0.05).During cesarean section,the heart rate and systolic blood pressure in the two groups were increased,and the increase was more significant in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in SAS score between the two groups at admission(P>0.05).Postoperative SAS score of observation group was significantly lower than control group,physiological and psychological comfort Score was significantly higher than control group,and the difference was statistically significant(P<0.05);the nursing satisfaction of patients in the observation group was 91.67%,which was significantly higher than that in the control group 72.22%,and the difference was statistically significant(P<0.05).In the observation group,9 doctors were very satisfied with the nursing,3 were satisfied,and 1 was not satisfied;in the control group,4 doctors were very satisfied with the nursing,4 were satisfied,2 were not satisfied,and 3 were not satisfied,and the difference was statistically significant(U=2.064,P=0.039).Conclusion The refined care model in PPP caesarean section can effectively shorten the operation time,reduce intraoperative and postoperative blood loss,stabilize the patient's heart rate and systolic blood pressure,and improve patient anxiety.At the same time,it improves the psychological and physical comfort of patients,reduces the incidence of postoperative complications,and improves the satisfaction of doctors and patients.
