摘要
目的研究丹参多酚酸盐对急性心肌梗死患者缺血心肌的保护作用及血流动力学的影响。方法选取2017年10月-2018年9月在河北北方学院附属第二医院诊治的80例急性心肌梗死患者为研究对象,采用随机数字表法分为对照组和观察组,各40例。对照组采用阿司匹林+替格瑞洛治疗,观察组在对照组基础上联合丹参多酚酸盐治疗,比较两组患者心肌指标水平、抗氧化功能指标水平、血流动力学指标、炎症指标水平以及临床不良反应。结果观察组cTnI、CK-MB均低于对照组(P<0.05);观察组GSH-Px、SOD均高于对照组,MDA低于对照组(P<0.05);观察组红细胞沉降率、红细胞比容、纤维蛋白原、高切全血黏度、低切全血黏度均低于对照组(P<0.05);治疗后,观察组hs-CRP、IL-6、NT-proBNP均低于对照组(P<0.05);观察组不良反应发生率为5.00%,与对照组的7.50%比较,差异无统计学意义(P>0.05)。结论丹参多酚酸盐治疗急性心肌梗死患者可有效保护心肌组织,降低炎症指标,改善血流动力学指标,提高抗氧化能力,减轻氧化反应心肌缺血再灌注损伤,临床应用效果确切。
Objective To study the protective effect of salvianolate on ischemic myocardium and hemodynamics in patients with acute myocardial infarction.Methods A total of 80 patients with acute myocardial infarction who were treated in the Second Affiliated Hospital of Hebei North University from October 2017 to September 2018 were selected as the research objects.They were divided into control group and observation group by random number table method,with 40 cases in each group.The control group was treated with aspirin and ticagrelor,and the observation group was treated with salvianolate on the basis of the control group.The levels of myocardial indexes,antioxidant function indexes,hemodynamic indexes,inflammatory indexes and clinical adverse reactions were compared between the two groups.Results cTnI and CK-MB in the observation group were lower than those in the control group(P<0.05).GSH-Px and SOD in the observation group were higher than those in the control group,MDA was lower than that in the control group(P<0.05).he erythrocyte sedimentation rate,hematocrit,fibrinogen,high-shear whole blood viscosity and low-shear whole blood viscosity of the observation group were lower than those of the control group(P<0.05).After treatment,hs-CRP,IL-6 and NT-proBNP in the observation group were lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was 5.00%,which was compared with 7.50%in the control group,the differences was not statistically significant(P>0.05).Conclusion Salvianolate in the treatment of patients with acute myocardial infarction can effectively protect myocardial tissue,reduce inflammatory indexes,improve hemodynamic indexes,improve antioxidant capacity,and reduce myocardial ischemia-reperfusion injury caused by oxidative reactions.The clinical application effect is accurate,which has definite clinical application effect.
