摘要
目的探讨药师在线审方在碳青霉烯类抗菌药合理使用及规范管理中的应用效果。方法收集2018年1月至2019年12月菏泽市立医院使用碳青霉烯类抗菌药物的患者信息,依据是否实施药师在线审方分为审方前(2018年1—12月)和审方后(2019年1—12月)两个阶段,并对两个阶段碳青霉烯类抗菌药物使用情况、使用合理性及用药管理等指标进行比较分析。结果审方后阶段我院碳青霉烯类抗菌药物的使用量与使用金额总体略有增加;使用率和碳青霉烯类占抗菌药物使用人数比例较审方前阶段下降(P<0.05)。审方后阶段的碳青霉烯类抗菌药的使用及管理较审方前阶段改善明显,其中适应证不适宜病例占比、用药前无微生物送检、用法用量不适宜、品种选择不正确分别较审方前阶段下降11.25%、8.75%、17.50%、5.83%,差异有统计学意义(P<0.05),联合用药不适宜和用药疗程不适宜分别较审方前阶段下降1.66%、0.83%,差异无统计学意义(P=0.218,0.218);审方后阶段的会诊率、治疗效果评估以及病程记录中用药分析分别提升14.17%、9.17%、15.83%,差异有统计学意义(均P<0.05)。结论药师在线审方可有效促进我院碳青霉烯类抗菌药的合理使用和规范管理,应当被推广应用。
Objective To explore the application effect of online prescription review by pharmacists in the rational use and standardized management of carbapenem antibacterial drugs.Methods The information of patients using carbapenem antibacterial drugs in Heze Municipal Hospital from January 2018 to December 2019 was collected and divided into the stage before prescription review(from January to December 2018)and the stage after prescription review(from January to December 2019)according to whether online prescription review by pharmacists was implemented.The indicators of carbapenem use,rationality of use and medication management in the two stages were compared and analyzed.Results The use amount and cost of carbapenem in our hospital were generally slightly increased in the stage after prescription review.The use rate and patient proportion using carbapenem of antimicrobial drugs were decreased in the stage after prescription review than those in the stage before prescription review(P<0.05).The use and management of carbapenem in the stage after prescription review were improved significantly those in the stage before prescription review,with a decrease of the proportion of cases with inappropriate indications,no microbiological examination before use,inappropriate usage and dosage as well as incorrect variety selection by 11.25%,8.75%,17.50%and 5.83%,respectively,and there were statistically significant differences(P<0.05).The inappropriate combination medication and inappropriate medication course were decreased by 1.66%and 0.83%respectively in the stage after prescription review than those in the stage before prescription review,with no statistically significant differences(P=0.218,0.218).The consultation rate,assessment of therapeutic effect and medication analysis in the disease course record were increased by 14.17%,9.17%and 15.83%respectively in the stage after prescription review,with statistically significant differences(all P<0.05).Conclusion Online prescription review by pharmacists can effectively promote the rational use and standardized management of carbapenem antibacterial drugs in our hospital,which is worthy of promotion and application.
