摘要
目的探讨抗血管内皮生长因子(VEGF)药物联合激光治疗成人型Coats病的临床效果。方法选取26例(26眼)成人型Coats病患者,所有患者均采用抗VEGF药物联合激光进行治疗。分析患者全身及局部并发症发生情况、黄斑和水肿改善情况,对比治疗前后最佳矫正视力(BCVA)、视网膜中心厚度(CRT)。结果治疗过程中,患者未出现眼压增高、白内障情况加重、眼内炎等并发症,整个手术过程顺利完成。治疗前,患者BCVA为(51.15±15.14)个字母,治疗1周、1个月、3个月及末次随访时BCVA分别为(63.89±13.88)、(67.87±10.09)、(68.06±10.84)、(67.52±6.64)个字母;治疗1周后、1个月、3个月及末次随访时BCVA均高于治疗前,差异具有统计学意义(P<0.05)。治疗1周、1个月、3个月及末次随访时CRT均小于治疗前,差异具有统计学意义(P<0.05)。26例患者中,3例患者出现了黄斑、水肿现象,治疗后患者黄斑、水肿得到显著改善。结论针对成人型Coats病应用抗VEGF药物联合激光治疗的临床效果显著,具有较高安全性,可有效提高患眼视力,减少视网膜积液。
Objective To discuss the clinical effect of anti-vascular endothelial growth factor(VEGF)drugs combined with laser in the treatment of adult Coats’disease.Methods 26 patients(26 affected eyes)with adult Coats’disease were treated with anti-VEGF drugs in combination with laser.The occurrence of systemic and local complications,macular and edema improvement of patients were analyzed,and the best corrected visual acuity(BCVA)and central retinal thickness(CRT)before and after treatment were compared.Results During the treatment,the patient did not experience complications such as increased intraocular pressure,aggravation of cataracts,and endophthalmitis,and the entire operation was successfully completed.BCVA of patients before treatment was(51.15±15.14)letters,and after 1 week,1 month,3 months of treatment and at the last follow-up were(63.89±13.88),(67.87±10.09),(68.06±10.84)and(67.52±6.64)letters respectively.After 1 week,1 month,3 months of treatment and at the last follow-up,BCVA was higher than that before treatment,and the differences were statistically significant(P<0.05).After 1 week,1 month,3 months of treatment and at the last follow-up,CRT was less than that before treatment,and the differences were statistically significant(P<0.05).Among the 26 patients,3 patients developed macular and edema.After treatment,the macula and edema of the patients were significantly improved.Conclusions Combination of anti-VEGF drugs and laser has significant effect on the treatment of adult Coats’disease,and it can effectively the vision and reduce retinal effusion of patients with high safety.
