摘要
目的观察一次性活检针行经皮胸部肿物穿刺活检的应用价值。方法搜集经胸部增强CT发现,性质未明或经过治疗后需要再次活检的肺部或纵隔肿物患者共72例,行超声引导(16例)或CT引导(56例)经皮粗针活检,取得组织学标本行病理学检查。结果 72例患者共行120次经皮穿刺活检,活检成功率为88.9%~95.2%,总体疾病诊断率为97.2%;并发症发生率为27.85%,主要是气胸(13.9%),少量咯血(9.7%),胸壁积气和纵隔积气各2例(2.8%),未出现胸膜反应、休克、胸腔大血管破裂出血、气体栓塞及肿瘤转移等严重并发症。结论影像引导下经皮胸部肿物粗针穿刺活检适应症广、可操作性强、疾病诊断率高、安全性好以及临床应用价值大,值得临床广泛应用和推广。
Objective To observe the application value of one-off biopsy needle in percutaneous biopsy of thoracic masses. Methods 72 cases of lung or mediastinal masses with unknown nature or need to be re biopsied after treatment were found by chest enhanced CT,Ultrasound-guided(16 cases) or CT-guided(56 cases)percutaneous needle biopsy were performed to obtain histopathological examinations. Results 72 patients underwent 120 percutaneous biopsies, the successful rate of biopsy was 88.9%-95.2%,the overall diagnosis rate was 97.2%;the incidence of complications was 27.85%,mainly pneumothorax(13.9%),a small amount of hemoptysis(9.7%),pneumothorax and mediastinum(2.8%),and no pleural reaction, shock, rupture and hemorrhage of thoracic great vessels, gas embolism and tumor metastasis occurred. Conclusion Image-guided percutaneous biopsy of thoracic masses has wide indications, strong operability, high diagnosis rate, good safety and great clinical application value, which is worth wide application and promotion in clinical practice.
